Wahlen VA Salt Lake City Health Care System Pathology & Laboratory Medicine Service, 500 Foothill Drive (113), Salt Lake City, UT 84148. Services shall be required for a base year period beginning August 1, 2017 through July 31, 2018 with four one-year options for renewal thereafter in strict accordance with all schedules, specifications, terms, conditions and provisions of this contract. CONFORMANCE STANDARDS All services provided under this contract must be performed in conformance with the Federal Drug Administration (FDA), Occupational Safety and Health Administration (OSHA), College of American Pathologist (CAP), and Original Equipment Manufacturer standards and specifications. HOURS OF WORK Hours of work for preventive maintenance and repairs are defined as Monday through Friday from 8 a.m. to 5 p.m., excluding federal holidays or as otherwise arranged with the Contracting Officer Point of Contact (POC) or the Pathology Lead in the Histo/Cyto/Immunohistochemistry department at the VA SLC department. Contractor is not responsible to provide services on the following federally recognized holidays: New Year s Day Labor Day Martin Luther King s Day Columbus Day President s Day Veteran s Day Memorial Day Thanksgiving Day Fourth of July Christmas Day Also, any other day declared by the President of the United States to be a national holiday. 4. PREVENTIVE MAINTENANCE: a. The Contractor shall perform two annual preventive maintenance procedures during each year of the contract as arranged with the Point of Contact (POC) or the Pathology Lead in the Histo/Cyto/Immunohistochemistry department at the VA SLC department. Preventive maintenance procedures shall be in accordance with the published preventive maintenance manuals for the equipment listed in the schedule. The Contractor shall utilize the Original Equipment Manufacturer's established procedures and checklists, (or Contractor-supplied equivalent satisfactory to the Contracting Officer POC or the Pathology Lead in the Histo/Cyto/Immunohistochemistry department at the VA SLC department. b. A Field Service Report shall be supplied to the Contracting Officer POC at the completion of each preventive maintenance procedure or repair service call. c. Repair service call report shall include, but is not limited to, the following: date and time notified date and time of arrival serial number, type, and model number of equipment time spent for repair description of malfunction and repair proof of repair that includes documentation of a sample run of quality control verifying acceptable performance Preventative maintenance report shall include, but is not limited to, the following: Cleaning of equipment Completing Original Equipment Manufacturer field service updates for operational and reliability change notices Calibrating and lubricating the equipment Performing remedial maintenance of non-emergent nature Testing and replacing faulty and worn parts Inspecting/replacing tubing for wear and fraying Inspecting all mechanical components including, but not limited to; cables and mounting hardware, chains, belts, bearings and tracks, interlocks, and motors for mechanical integrity, safety, and performance to Original Equipment Manufacturer specifications Returning the equipment to operating condition defined in Original Equipment Manufacturer specifications Replacing any Original Equipment Manufacturer labels, decals, or warning tags that are not legible Providing documentation of services performed The Contractor shall notify the Contracting Officer POC or the Pathology Lead in the Histo/Cyto/Immunohistochemistry department at the VA SLC department of the existence or development of any defects in, or repairs to the equipment covered under this contract which the Contractor considers he/she is not responsible for under the terms of the contract. All exceptions to the Preventive Maintenance Inspection schedule shall be arranged and approved in advance with the Contracting Officer POC or the Pathology Lead in the Histo/Cyto/Immunohistochemistry department at the VA SLC department. The contractor shall not leave a service call open pending verification that repairs are completed by the contractor s maintenance personnel. The contractor shall treat each notification for an emergency/repair service call from the Government as a separate and new service call. Any components/parts utilized to perform preventative maintenance must meet manufacturer's specifications, be noted on the service slip and have a 90 day warranty. Refurbished replacement parts shall not be acceptable. EMERGENCY REPAIR SERVICE: a. Emergency repairs shall be performed after initial telephone notification that the equipment is inoperative. The contractor shall provide the Government with a designated point of contact and telephone number and shall make arrangements to enable his maintenance representative to receive such notification. b. The contractor shall be on-site at the George E. Wahlen VA Salt Lake City Health Care System Pathology & Laboratory Medicine Service to perform emergency repair service to the LEICA BOND MAX (S/N M212270), VA ID#: EE 63447) within twenty-four (24) hours after receipt of initial telephone notification of a malfunction Monday through Friday 8 a.m. to 5 p.m. Contractor shall provide coverage for repairs Monday through Friday from 8 a.m. to 5 p.m., excluding federal holidays or as otherwise arranged with the Contracting Officer POC. c. Contractor shall provide technical support service to assist in troubleshooting 24 hours per day, 7 days per week including Federal holidays. d. Any components/parts utilized to make repairs must meet manufacturer's specifications, be noted on the service slip and have a 90 day warranty. Repaired or refurbished replacement parts shall not be acceptable. PARTS The Contractor shall furnish all parts necessary to perform the preventative maintenance or emergent repair on the equipment, covered by this contract, in accordance with the Conformance Standards Section (Section II). The Contractor stipulates that he/she has ready access to new standard parts (manufactured, supplied by the manufacturer, or equal thereto). All parts supplied shall be of current manufacture and have full compatibility with existing equipment. Documentation of intended parts source(s) shall be provided to the Contracting Officer POC upon request. NOTE: Any additional charges outside the scope of this contract to be claimed must be approved by the Contracting Officer. The Contracting Officer Technical Representative or the Pathology Lead in the Histo/Cyto/Immunohistochemistry department at the VA SLC department will also contact the Contracting Officer POC prior to services being rendered. The contractor is responsible to have in-stock or have the ability to provide all necessary parts, components, assemblies, modules and equipment to restore any malfunctioning equipment to full performance within 24 hours of the originating call. Full performance is defined as all defective parts having been replaced with parts equivalent to or better than the original parts and meets or exceeds the manufacturer's original performance specifications. The contractor shall supply and bear the cost of all parts, lubricants, cleaning supplies and tools required for preventive maintenance and, as a result of normal wear and tear, restore equipment to full performance. NOTE: Parts, considered to be the manufacturer's recommended parts, are to be replaced in the course of a recommended preventive maintenance inspection. All parts shall meet the original manufacturer's design specification COMPETENCY OF PERSONNEL SERVICING EQUIPMENT The Contractor's personnel shall include a certified and experienced Field Service Engineer capable of making any required repairs or performing preventative maintenance on the LEICA BOND MAX (S/N M212270), VA ID#: EE 63447). Certification is based upon successful completion of the formalized training program for the equipment covered under this contract. For experience, the Field Service Engineers must have a minimum of one (1) year of experience providing preventive maintenance and emergency repair services on the same make and model of equipment covered under this contract. The contractor shall provide written assurance of the competency of their personnel by detailing a list of Field Service Engineers along with applicable training and years of service experience on the specific devices covered in this agreement. This information shall be included in the quoted proposal for providing service. TEST EQUIPMENT Upon request of the Contracting Officer's POC or the Contracting Officer, the Contractor shall provide a copy of the current Calibration Certification of all test equipment which is to be used by the Contractor to perform service under this contract. Calibration of equipment shall be traceable and in conformance with test equipment Original Equipment Manufacturer standards. SAFETY REQUIREMENTS: In the performance of this contract, the Contractor shall take such safety precautions as the Contracting Officer may determine to be reasonably necessary to protect the lives and health of occupants of the building. The Contracting Officer shall notify the Contractor of any safety issues and the action necessary to correct these issues. Such notice, when sewed on the Contractor or his representative at the work site shall be deemed sufficient for the corrective actions to be taken. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or part of the work, and hold the Contractor in default. CONTRACTOR PERSONNEL Contractor personnel shall present a neat appearance and be easily recognized as contractor employees. At the completion of work being performed, the contractor shall be responsible for cleaning up the work area and equipment. All accumulated rubbish shall be removed and disposed of in the appropriate waste receptacles. REPORTING REQUIREMENTS The Contractor shall check in with the Contracting Officer's POC or the Pathology Lead in the Histo/Cyto/Immunohistochemistry department at the VA SLC department prior to any services. When the service is completed, the FSE (Field Service Engineer) shall document services rendered on a legible ESR(s) (Electronic Service Report). The FSE shall check out with Contracting Officer's POC or the Pathology Lead in the Histo/Cyto/Immunohistochemistry department at the VA SLC department. All ESR(s) shall be submitted to the equipment user for an acceptance signature and then to the POC for an authorization signature. If the POC is unavailable, the Pathology Lead in the Histo/Cyto/Immunohistochemistry department at the VA SLC department may sign for the POC after confirming work is completed and the equipment is operational. EQUIPMENT DOWNTIME The contractor shall be liable to the Government for losses of production due to significant equipment downtime. Significant downtime is defined as seven (7) or more consecutive nonproductive/nonoperational days, including the weekend. Downtime records shall be maintained by both the POC and maintenance contractor. Equipment downtime is calculated based on the number of normal hours of coverage that the equipment is not fully operational. Downtime will begin when the contractor is required to be on site after notification by the CO, POC, or designated alternate. Downtime will accumulate until the scheduled equipment is returned to full operational condition and accepted as such by the POC or designated alternate. This does not include scheduled maintenance for Preventive Maintenance purposes or delay precipitated by the VA. Monies will be subtracted from the contract if the contractor fails to meet the uptime requirement as stated above. The amount computed will be 50% of the quarterly invoice payment for that quarter. ADDITIONAL CHARGES There will be no additional charge for time spent at the site during or after the normal coverage area awaiting the arrival of additional FSE and/or delivery of parts. VA REGULATIONS Contractor employees shall wear visible identification at all times when on premises of the VAMC. Contractor personnel are responsible for parking in appropriate designated parking areas. The VA will not be responsible for parking violations incurred by the contractor. Information on parking is available from the VA Police or the Pathology Lead and staff. Smoking is prohibited inside any VA facility. Contractors will adhere to signs posted at the VAMC relating to this facilities public safety requirements. Possession of firearms is prohibited. Enclosed containers, including tool boxes, may be subject to search at the discretion of the VAMC. INFECTION CONTROL Contractor shall conform to the infection control measures specified by the VAMC and manufacturers preventive maintenance procedures for the equipment. Preventive Maintenance Service will normally be performed in the work area and is subject to infection control procedures. CONTRACT SECURITY Contractors, contractors personnel, subcontractors, and subcontractor personnel shall be subjected to the same Federal laws, regulations, standards, and VA Directives and Handbooks as VA and VA personnel regarding information and information system security. The Contractor shall ask the CO, POC, Biomedical Engineering Department technician, or designated representative when in question about information security regarding VA information.
