- Quick Guide for International Entity Registration in SAM Request for Proposal SUP30017R0019 LE Health Insurance Services - WORD FORMAT Request for Proposal SUP30017R0019 LE Health Insurance Services - PDF FORMAT Invitation Letter The United States Embassy in Kyiv, Ukraine has a requirement for Health Insurance Services for its Locally Employed Staff and their eligible family members. The resultant contract will be a fixed-price Indefinite-Delivery-Indefinite Quantity for a base year, with four one-year option periods, if exercised by the Government. The Embassy intends to conduct a pre-proposal conference on July 18, 2017 at 14:00 at 4 Ihor Sikorsky St., Kyiv, Ukraine, and all prospective quoters are invited to attend. See Section L of the attached Request for Proposal. For pre-proposal conference registration, please provide your company contact information and names of visitors no later than 12:00 on July 17, 2017 to KyivGSO@state.gov. The deadline for receipt of offers is 09:00 a.m. Eastern time July 31, 2017 (04:00 p.m. local time July 31, 2017). No electronic offers will be accepted. Please mail your offer to the contracting office listed in this notice prior to the response date and time. The U.S. Government intends to award a contract to the responsible company submitting an acceptable offer at the lowest price. We intend to award a contract based on initial proposals, without holding discussions, although we may hold discussions with companies in the competitive range if there is a need to do so. Please note: Proposals that contain more benefits (even if there is no increase in cost) or fewer benefits than those stated in the solicitation may be deemed technically unacceptable. All contractors have to be registered in the SAM (System for Award Management) Database https://www.sam.gov prior to contract award pursuant to FAR provision 5.207. Therefore prospective offerors are encouraged to register prior to the submittal of quotations/proposals. A Quick Guide for International Entity registration in SAM is attached to this announcement. The guidelines for registration in SAM are also available at: https://www.statebuy.state.gov/pd/Pages/AdditionalVendorReg.aspx According to the Law of Ukraine “On Changes to the Law of Ukraine “On Insurance” #2745-14 dated June 28, 2015 (Chapter I, Article 2, and Chapter IV, Article 38), only companies which are residents of Ukraine and are properly licensed in Ukraine, are allowed to provide insurance services in Ukraine. Companies must obtain and/or provide proof of all permits and licenses required to operate as a health insurance provider in Ukraine.
