Description
This is a SOURCES SOUGHT request for informational and planning purposes only and shall not be construed as a solicitation or as an obligation or commitment by the Government at this time. This notice is intended strictly for market research. The purpose of this Sources Sought notice is to determine the following: The interest and capability of all qualified business sources; Whether qualified business sources are small business, women-owned small business, small disadvantaged business, veteran owned small business, service-disabled veteran owned small business, or large business. The Contracting Office is particularly interested in identifying service disabled veteran owned small business (SDVOSB) and veteran owned small business (VOSB); Whether the business source is a manufacturer or authorized distributor, verified by an authorization letter from the original equipment manufacturer; and Sized classification relative to the North American Industry Classification System (NAICS) code 334290 for the proposed acquisition. The Department of Veterans Affairs, Network Contracting Office 10 is seeking potential sources for an appointment kiosk system to be provided for the Saint Joseph County VA Clinic Pharmacy, 1540 Trinity Place, Mishawaka, IN 46545. The system shall include all hardware, software, displays, installation and training, and maintenance for a period of one year. The proposed system shall consist of the following characteristics and concept of operations, although may vary slightly from brand to brand: Touch Screen Kiosk QTY: 1 Prescription Notification Board QTY: 3 Central Platform (Server and Datapoint) UPS Battery Backup No PC Required Pharmacy Data Terminal Ticket Window Display ( Now Serving to go above each counseling room) QTY: 2 Kiosk Stand Audio Speaker Package Integration System with Audio and Notice Board Standard Interface Installation, Setup, and On-Site Training Virus Protection Interface System Maintenance for 1 Year Concept of Operations: Patient checks in upon arrival at free standing Kiosk. Kiosk provides a number. Notice board can also display video information, operating hours, etc. Kiosk can manage both scheduled appointments and walk-ins reducing wait times. Appointments are moved ahead in the que as determined by their appointment time. All contractors that are interested in meeting this requirement are asked to submit the following information: Company Name Company Address DUNS Number Company POC Name Company POC Phone Company POC Email Business size/type under NAICS 334290 GSA Contract Number (if applicable) Additional information: All contractors must be registered with the System for Award Management (SAM) at http://www.sam.gov. Any contractor that believes they are capable and desires to claim preference for small business status must be registered with the SBA and meet the requirements of FAR 19.102. Any contractor that believes they are capable and desires to claim preference for veteran owned small business status must be registered with the Vendor Information Pages (VIP) at https://www.vip.vetbiz.gov as an SDVOSB or VOSB.
