Description
This is a Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding: (1) the availability and capability of qualified sources; and (2) whether they are Native-owned businesses; small businesses; HUBZone small businesses; service-disabled, veteran-owned small businesses; 8(a) small businesses; veteran-owned small businesses; woman-owned small businesses; or small disadvantaged businesses. The North American Industry Classification System (NAICS) code for the proposed acquisition is 541990 (Other Professional, Scientific, and Technical Services) with size standard of $15.0M. Your responses to the information requested will assist the Government with its acquisition set-aside determination. BACKGROUND The Indian Health Service (IHS) Improving Patient Care Program is to transform the health care system by improving the quality of and access to care for American Indian/Alaska Native people. The Patient Centered Medical Home (PCMH) model of care processes have been integrated into IPC improvement work as a framework for system wide quality improvement. The IHS plans to expand this work to assist all IHS ambulatory care facilities, including hospital based ambulatory care, health centers and health clinics to attain a PCMH designation in the Indian health system. The ultimate goal is to provide the support needed by IHS ambulatory care facilities to achieve PCMH recognition and designation, and to benchmark levels of quality improvement capabilities, efficiency and patient experience. The purpose of the acquisition is to provide technical assistance and training for the IPC care teams, the National IPC Team, and faculty from across the Indian health system to provide the support needed by IHS ambulatory care facilities to achieve PCMH recognition and designation, and to build capability in quality improvement science skills and knowledge to develop high-performing, innovative health care teams to improve the quality of and access to care. The Indian Health Service (IHS), Improving Patient Care (IPC) Program, has a need for services to include training and technical assistance for National IPC staff as well as care teams from Indian Health Service, Tribal and Urban (I/T/U) facilities in attaining PCMH designation, and additional quality improvement science training. The prospective contractor needs to have the following capabilities, performance characteristics or standards in order to meet the requirements for this acquisition: Training and technical assistance for PCMH teams, Ongoing training and technical assistance for the IPC National Team, develop system and disease-specific Intensives (modules), assist IPC National Team with data analysis and PCMH-A data analysis, disseminate improvement throughout the Indian Health system through IPC-MS, provide reports on progress by PCMH sites, • PCMH training and technical assistance for IPC teams, develop a Leadership track for IPC, re-brand and restructure the Improvement Support Team and their roles and responsibilities, and provide additional and augments Quality Improvement Training to meet the changing needs of IPC and efforts to achieve PCMH designation which will provide support needed by IHS ambulatory care facilities to achieve PCMH recognition and designation, and to build capability in quality improvement skills to develop high-performing, innovative health care teams to improve the quality of and access to care. The award will consist of one base year with four-option years. The anticipated Period of Performance will be from time of award to July 2022. RESPONSE INFORMATION THE CAPABILITLY STATEMENT PRESENTED MUST CLEARLY DEMONSTRATE THE CONTRACTOR'S EXPERTISE AND EXPERIENCE RELATING TO providing technical assistance and training to support ambulatory care facilities, capability to improve science skills that will develop high-performing and innovative teams to improve quality of and access to care. Responses SHALL ADDRESS EACH OF THE FOLLOWING INFORMATION to be considered a complete and compliant response. 1. Contractor's technical knowledge, understanding, and competence(s) related to the requirements. 2. Contractor's recommended approach to perform the work. 3. Contractor's past performance and experience. 4. Contractor's management approach, specifically in relation to quality control and quality assurance. INSTRUCTIONS TO INDUSTRY All interested parties are encouraged to respond. Responses must directly demonstrate the contractor's capability, experience, and ability to marshal resources to effectively and efficiently perform the objectives described above. Generic capability statements are not sufficient and will not be considered compliant with the requirements of this notice. The Government requests interested parties submit a written response to this notice which includes the following information: a. Company Name b. Company point of contact, mailing address, and telephone number(s), and website address c. Name, telephone number, and e-mail address of a company point of contact who has the authority and knowledge to clarify responses with government representatives d. Company GSA Schedule number or other available procurement vehicle (if applicable) e. Company's System for Award Management (SAM) registration status. All eligible respondents to the upcoming Request for Proposal (RFP) must be registered on System for Award Management (SAM), http://www.sam.gov/. Disclaimer and Important Notes. This notice does not obligate the Government to award a contract. Any information provided by industry to the Government as a result of this sources sought synopsis is strictly voluntary. Responses will not be returned. No entitlements to payment of direct or indirect costs or charges to the Government will arise as a result of contractor submission of responses or the Government's use of such information or otherwise pay for the information provided in response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After a review of the responses are received, a pre-solicitation synopsis and solicitation may be published on the Government Point of Entry (www.fbo.gov). However, responses to this notice will not be considered adequate responses to a solicitation. Confidentiality. No proprietary, classified, confidential, or sensitive information shall be included in your response. The Government reserves the right to use any non-proprietary technical information provided by the contractor in any resultant solicitation(s). Questions must be submitted via e-mail as directed below, no later than 4:00 p.m. (EST) on July 5, 2017. Reponses to this sources sought notice are due on 4:00 p.m. (EST) on July 13, 2017. Capability statements will not be returned and will not be accepted after the due date. The maximum number of pages for this submission is fifteen (15) pages.
