- Air Force Test Center, Installation Contracting, Eglin AFB, FL, intends to solicit and award a sole source firm-fixed price contract for Sierra Summit 12 Channel System (EMG NCV machine) to Cadwell Laboratories, Inc. of Kennewick, WA Cage: 5XLB7, in accordance with FAR 13.106-1 CLIN 0001: Sierra Summit 12 Channel System (EMG NCV machine) Sierra Summit 12 Channel System, EMG, NCV, F/H, RNS, Blink and RR interval, SEP (Somatosensory Evoked Potentials), Visual Evoked Potential (VEP) with 22" LED Monitor, DVI-to HDMI cable 15', All in one Desktop PC (open market), USB Cable for Desktop PC 6'(open market), Microsoft Office, T2 Cart (assembled) Summit 3-12 Ch Arm with Stim, Amp and needle holders, Arm Mount for T2 Cart (5 lb), Cable Holder, Isolation Transformer 800VA 110V/22C (6 inputs), Laserjet Printer (open market), GH Electrode Kit, Stimulator Probe, Straight with touchproof connectors and round head, large diameter, USB Footswitch, Temperature Probe, Electronic Technical Manual, USB, Level 2 Service Contract 3-12 Ch (in addition to standard 1 year warranty) The North American Industry Classification System (NAICS) for this acquisition is 334510, small business size standard: 1250 employees. Interested parties may identify their interest and capability to respond to the requirement. This notice is not a request for competitive proposals/quotes. However, the government will consider all responses received by 05 July 2017 by 03:00 P.M. Central Standard Time. A determination by the government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the government.
