- Sources Sought Notes This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this sources sought must be in writing. The purpose of this sources sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service Disabled Veteran Owned Small Businesses and Veteran Owned Small Businesses interested and capable of providing the services described below. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. Responses to this notice shall include the following: (a) company name (b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number (h) Type of small business, e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business and (i) must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below and/or a letter stating that you are a distributor for this product from manufacturer. Only electronic responses are acceptable, sent by email to theresa.hamilton@va.gov Important information: The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. The VA is mandated by Public Law 109-461 to consider a total set-aside for Service Disabled Veteran Owned Small Business set aside. However, if response by Service Disabled Veteran Owned Small Business firms proves inadequate, an alternate set-aside or full and open competition may be determined. No sub-contracting opportunity is anticipated. The North American Classification System (NAICS) code for this acquisition is 339112 (1,000 employees) Surgical and Medical Instrument Manufacturing. Notice to potential offerors: All offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM) at www.sam.gov and complete Online Representations and Certifications Application (ORCA). Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov. All interested Offerors should submit information by e-mail to. All information submissions shall be received no later than 5:00pm Eastern Time on Monday, July 10th, 2017. State of Work Spider2 Limb Positioner General Objectives and Requirements Purpose: The Phoenix VA Health Care System (Agency) needs to replace (1) limb positioner used in arthroscopic surgeries. A complete set includes (1) limb positioner, (1) battery pack, and (1) transport cart. Background: The Agency uses Smith & Nephew SPIDER limb positioners to keep the shoulders of patients in place during arthroscopic surgeries. The limb positioner is able to stably tilt and rotate the patient s arm. The limb positioner gets so much use that the post that attaches to the operating table and its arm extension are starting to fail. The manufacturer confirmed that the shoulder attachments the Agency is currently using with the SPIDER model are compatible with the SPIDER2 limb positioner. Scope and Methodology: The Contractor will deliver the requirement in its entirety as listed below: Contractor shall ship and deliver all requirements as listed Brand Name or Equal: (1) SPIDER2 limb positioner (1) SPIDER2 battery pack (1) SPIDER2 transport cart Materials will new, not refurbished. Contractor shall be a manufacturer authorized reseller/ distributor of the quoted items If the Contractor is quoting/offering or equal items, the Contractor must submit manufacturer product literature that confirms the item(s) meets all the salient characteristics for the specified items. The salient characteristics for the specified item(s) are in 3.G. of this SOW. If the Contractor is quoting/offering or equal items, the Contractor must clearly identify the exact make, model, manufacturer and item description for each line item to be considered. Quotes/offerors that do not provide an adequate description of how the equal product meets the salient characteristics will not be considered for award. This is not applicable to offerors that are quoting EXACT MATCH items. VAAR Part 852.246-70 Guarantee: The contractor guarantees the equipment against defective material; workmanship and performance for a period of 12 months, said guarantee to run from date of acceptance of the equipment by the Government. The contractor agrees to furnish, without cost to the Government, replacement of all parts and material that are found to be defective during the guarantee period. Replacement of material and parts will be furnished to the Government at the point of installation, if installation is within the continental United States or f.o.b. the continental U.S. port to be designated by the contracting officer if installation is outside of the continental United States. Cost of installation of replacement material and parts shall be borne by the contractor. Functional/Performance/Specification Statement and Evaluation Criteria NO. SPECIFICATION/CRITERIA RATING 1.
