Description

Statement of Work Television Channels Service for the Bath VA Medical Center This solicitation is for SERVICE DISABLED VETERAN OWNED SMALL BUSINESSES (SDVOSB) ONLY. Note: If claiming SDVOSB or VOSB, the firm must be registered and CVE verified in VetBiz Registry http://www.vetbiz.gov/; also, include VetBiz certification. Offers from unverified SDVOSBs or VOSBs are considered deficient and will not be evaluated. This is a "Base Year" agreement for the period of a Base Year, plus four (4) additional Option Years. Following contract option year awards will coincide with the Government's fiscal year for the consecutive years. Contract dates for this service will be as follows: Base Year : 10/01/2017 to 09/30/2018 Option Year #1: 10/01/2018 to 09/30/2019 Option Year #2: 10/01/2019 to 09/30/2020 Option Year #3: 10/01/2020 to 09/30/2021 Option Year #4: 10/01/2021 to 09/30/2022 Effective date of the contract, will be the DATE OF SERVICE ACTIVATION. SEE ATTACHED DOCUMENT, STATEMENT OF WORK, FOR FURTHER INFORMATION. On-site visit information: FBO NOTICE VA242-17-Q-0837 VA242-17-AP-4197 - BATH NY VAMC TV SERVICE BASE YEAR + 4 Option Years On-site visit: All offerors will be given the opportunity to perform an on-site visit prior to submitting a quote for review. The date for this on-site visit is SCHEDULED FOR FRIDAY JULY 14, 2017 AT 9:00 AM. Please meet at the 1st floor center lobby, VA Medical Center, Bath, NY. If you should need to call for directions or notify of travel difficulties, phone Curt Rudolph at 607-329-7256. All questions or clarifications from the site visit need to be submitted by 3:00 PM Monday, July 17, 2017. Responses will be updated to the RFQ no later than 4:00 PM, Tuesday, July 18, 2017. The RFQ will close 4:00PM ET, Wednesday, July 19, 2017.  All quotes must be received by this deadline, to be considered.
