- REQUEST FOR INFORMATION: VA262-17-N-1301 THIS REQUEST FOR INFORMATION (RFI) IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. THE SUBMISSION OF PRICING, CAPABILITIES FOR PLANNING PURPOSES, AND OTHER MARKET INFORMATION IS HIGHLY ENCOURAGED AND ALLOWED UNDER THIS RFI IN ACCORDANCE WITH (IAW) FAR 15.201(e). This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified Service Disabled Veteran Owned Small Businesses (SDVOSB) or Veteran Owned Small Businesses (VOSB) for relative to NACIS 321992 sources (size standard 500 Employees). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published in the FedBizOpps or GSA eBuy websites. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this sources sought announcement. The Department of Veterans Affairs, Network 22 Contracts Office, is seeking sources to provide the supplies listed below. If you are interested, and are capable of providing the required supplies please provide the requested information as indicated below. Responses to this notice should include: company name, address, point of contact, size of business pursuant to the following questions: (1) Please indicate the size status and representations of your business, such as but not limited to: Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), Hubzone, Woman Owned Small Business (WOSB), Large Business, etc.)? (2) Is your company considered small under the NAICS code identified under this RFI? (3) Are you the manufacturer or distributor of the items being referenced above (or equivalent product/solution)? What is the manufacturing country of origin of these items? (4) If you re a small business and you are an authorized distributor/reseller for the items identified above (or equivalent product/solution), do you alter; assemble; modify; the items requested in any way? If you do, state how and what is altered; assembled; modified? (5) Does your company have an FSS contract with GSA or the NAC or are you a contract holder with NASA SEWP or any other federal contract? If so, please provide the contract type and number. CURRENT LEAD TIME IS 8 WEEKS ARO TERMS: NET 30 1 Job STATEMENT OF WORK (SOW) 1. Contract Title. K-9 Police Dog Kennel 2. Background. Horizon Structure has been assisting law enforcement and fire service agencies in all phases of emergency vehicle procurement and up fitting since our inception in 1995. As a single source supplier we supply agencies with everything from replacement parts and custom equipment fabrication to complete bumper to bumper equipment outfitting and installation. We also offer vehicle leasing and vehicle sales. Horizon Structure uses 5S methodology to improve quality, productivity, efficiency and safety throughout our installation process. The result of this process insures that we produce a more consistent finished product which allows us to warranty the work we do for the life of the vehicle. All work performed will meet or exceed the standards of any agency we work with 3. Scope. The building and installation of a canine kennel for two K-9 Police Dogs to be house at the VA Police Service Bldg. 236 and with it partner / trainer. These Police Dogs will be used for the sole purpose of law enforcement for the safety of the veterans, the employees who work on the VA facilities all who are conducting business at the Greater Los Angeles VA Healthcare System and the CBOC Clinics as needed. 4. Specific Tasks. This task is for the building of two (2), 8 x 10, a single K-9 unit. Task 1 - Enterprise Management Controls. 5.1.1 Subtask 1 - Integration Management Control Planning. Attached there is a copy of the Canine Unit 23, alone with the approval for the K-9 unit for the training and handling of the VA Police K-9 and the caring for the K-9, copy of the K9 kennel quote, IGCE, 2237, 5.1.2 Subtask 2 - Contract Management. See Attachment for the K-9 units plans for Building 236 K-9 unit double unit. Deliverables: Contract Management Plan Monthly Status Report 5.2.2 Subtask 2. Deliverables: 6. Performance Monitoring: This project will be monitored by Chief and Deputy Chief, Captain of Threat Management, the Physical Security Specialist, Fire and Safety, as well as the K-9 Unit Officer, Training Officers who oversees the K-9 project from beginning to end to ensure that specification is meet to be specified as stated by Policy 7. Security Requirements Since this project is to be done at the VA Police Service and it does not have anything to do with patients records, files rooms etc. there is no conflict with regarding confidentiality of patient records. 8. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). The only thing that I see that would involve any government equipment is the electricity that would use to power the kennel with lights, AC and heating. The two K9 units location will be determined by the VA Police Service Chief, Deputy Chief or the Captain. 9. Other Pertinent Information or Special Considerations. 1. See attachment for all the warranties that is covered under this project. 2. Install per strict Manufacturer s recommendations and requirements as to not void product warranty. 3. As builds / O & M Manuals Contractors shall provide as builds and O &M Manuals for this installation and the product used a. Identification of Possible Follow-on Work. b. Identification of Potential Conflicts of Interest (COI). The information that we use in trying to select a vendor all came from GSA. There is no one who is affiliated with, relative or otherwise in regarding to making our decision on obtaining a vendor for this project. c. Identification of Non-Disclosure Requirements. Everything with regards to this project and other relation to the project MUST be done per Policy as stated in the Canine Unit and SOP and VA Handbook 0730. The contractor d. Packaging, Packing and Shipping Instructions. The finished product, when delivered, from start to finished, should not be damaged in any way. Any damages done by the company need to be paid and replaced by the company itself. That includes every wood, every shingle, ever glass used, every inch of chain link fence. According to the quote that the VA Police received, the kennel units will be delivered to the customer area of choice of property to be determined by the VA Police Department e. Inspection and Acceptance Criteria. The K9 Units, once completed with be inspected by the Chief, Deputy Chief, Captain, K-9 10. Risk Control There is no risk that would involve patients, patients records, computer or network access of any kind. 11. Place of Performance. Greater Los Angeles Healthcare System, VA Police Service Bldg. 236, 11301 Wilshire Blvd, West Los Angeles, CA 90073 next to Police Service (Bldg. 236) 12. Period of Performance. The project time is about 8 to 10 weeks. The kennel will be delivered to the VA Police Service in West Los Angeles and ready for used once received. Project will begin once it has been awarded. It will be delivered via freight to customer property: Freight not to exceed $249.00.
