Description
1. This Amendment 1 revises the PWS as follows: a. PWS paragraph 6.10.5 is revised to read: 6.10.5 Long Term Monitoring and Long Term Operations and Maintenance The Contractor shall properly close and abandon three existing groundwater monitoring wells located on the flightline parking apron and prepare and submit well abandonment and closure reports. All monitoring well abandonment shall be completed in accordance with the State of Nebraska requirements by a State-licensed Driller and in accordance with the Air Force approved work plan. The groundwater monitoring wells are installed to a depth of 15 feet below grade and constructed using two (2)-inch diameter Schedule 40 polyvinyl chloride (PVC). Well abandonment activities shall include restoring the surface to match surrounding areas. (CDRL A001c) The contractor shall install temporary flightline barricades during well abandonment. The work area on the parking apron will be closed off with low-profile orange barricades. These barricades will be spaced no farther than 50 feet apart and will have red lights. The SS509 site is approximately 50 feet by 20 feet in area. The contractor shall use a high strength, fast setting concrete to abandon the wells. The concrete will be non-shrink, crack resistant, light grey in color and meet the following technical characteristics: • Initial set time of 15-minutes at 70°F • Compressive strength of 2,800-pounds per square inch (psi) 1-hour after final set; 3,600-psi after 3-hours; 5,000-psi after 7-days; and 6,000-psi after 28-days • Flexural strength of 420-psi 2-hours after final set; 650-psi after 1-day; and 750-psi after 28-days Reporting activities shall include the following (CDRL A001c): • Driller's report documenting the well abandonment activities submitted to the State of Nebraska; • Well abandonment report submitted to the Air Force that includes well abandonment logs and a copy of the Driller's well abandonment report; and • ERPIMS submittal documenting closure of the three wells. b. PWS Paragraph 7.0 Service Summary is revised as follows: PO 1: PWS Paragraphs 4.5 and 4.8.1. and 6.10 are revised to read as 4.8, 4.11, and 6.10. PO 3: PWS Paragraph 4.5 is revised to read as 4.11. PO 4: PWS Paragraph 4.8.1 is revised to read as 4.8. 2. The proposal due date of 11 July 2017, 2:00 PM Central Time, is extended to 18 July 2017 2:00 PM Central Time. 3. All other terms and conditions remain unchanged.
