- Brand Name Letter Justification Statement of Requirement/Item Description Solicitation SOLICITATION VERBIAGE This is a solicitation notice for a procurement of a MFC Hose Replacement as a 100% small business set aside requirement. FOB Destination is Tinker AFB OK. Quotes will be accepted on or before the closing date state on page 1 of the solicitation. No late quotes can be accepted. When return this form, please complete: Please complete Company Information (Name & Address) and CAGE Code in Block 17a. Please wet sign, put the name and title of signer, and date signed in blocks 30a., 30b., and 30c. respectively. Please insert pricing on Pages 3-11 along with net amount & delivery times. Please complete information in ALL “ Fill-in ” provisions and clauses. Not all provisions and clauses appear in the Rep/Certs under the company information in SAM. Per instructions, please review each page of the Solicitation and mark/input accordingly. Especially: a.FAR 52.204-17 b. FAR 52.209-2 c. FAR 52.212-3 d. FAR 52.225-18 e. AFFARS 5352.223-9000 (c) Please provide a written statement to explain how your company is going to meet the technical evaluation criteria listed in FAR Clause 52.212-2. It is the responsibility of the contractor to monitor FBO on a regular basis for any amendments. System for Award Management (SAM) registration is required to receive any award. Contractors can register electronically at http://www.sam.gov. The NAICS code for this requirement is 332912. With a size standard of 1,000.
