Exhibit C Supply List W81XWH-17-R-0009 Exhibit B, Past Performance Questionnaire W81WH-17-R-0009 Exhibit A, Pricing Sheet Solicitation W81XWH-17-R-0009 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposal are being requested and a written solicitation will not be issued. The following action is to be competed by the U.S. Army Medical Research Acquisition Activity (USAMRAA) in support of the U.S. Army Medical Research Institute of Chemical Defense (USAMRICD), Aberdeen, MD for General Laboratory Supplies, Chemicals, Scientific and Laboratory Equipment, and Animal Supplies. This announcement constitutes the solicitation by USAMRAA, Fort Detrick, MD as a Request for Proposal (RFP) and offers must be submitted in writing (oral offers will not be accepted). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-95; 19 January 2017. This acquisition is full and open competition in accordance with FAR 6.1. The North American Industry Classification System (NAICS) code is 339113 - Surgical Appliance and Supplies Manufacturing with a size standard of 750 employees. This acquisition is a Multiple-Award (MA) Blanket Purchase Agreement (BPA). The anticipated ordering period for the BPA is five (5) years. See the Attachments A through G for a list of the required supplies.
