Description
This is not a solicitation announcement. This is a Sources Sought publication only. The purpose of this Request for Information (RFI) is solely for information and planning purposes and does not constitute a solicitation. Please do not submit a proposal in response to this RFI. This notice is to assist the VA in the determination of sources only. A solicitation is not currently available. Responses to this sources sought is not a request to be added to a prospective bidders list or to receive a copy of the solicitation. The Department of Veterans Affairs (VA), Central Iowa Health Care System, at the outpatient clinic located in Fort Dodge, Iowa is seeking potential Contractors capable of providing a 1.0 Full-Time Equivalent (FTE) Board Certified /Board Eligible Internal Medicine or Family Practice Physician on site at the Fort Dodge Clinic, 2419 2nd Avenue N., Fort Dodge, Iowa 50501. These services are solely dedicated to Veteran patients of the Central Iowa VA Health Care System. The Fort Dodge VA CBOC is looking for responses from contractors regarding their capability providing 1.0 FTE Internal Medicine or Family Practice Physician Services. 1.0 FTE is defined as 40 hours per week; 2080 hours per year. The Fort Dodge CBOC is open Monday through Friday 8:00 AM to 4:30 PM, overtime may be available. Services to be provided include: Contractor s physician (s) (as appropriate and within scope of practice/privileging) shall be responsible for providing primary care, including, but not limited to: Board Certified/Board Eligible Internal Medicine/Family Practice Physician to see general practice patients eligible with the Department of Veterans Affairs. Contractor s physician(s) shall provide clinic primary care services. Contractor s physician(s) shall be present on time for any scheduled patient appointments as documented by physical presence in the clinic. Number of patients per clinic: 10-12 patients per day Clinical reminders with an emphasis on preventative healthcare Excellent staff and patient communication May conduct telemedicine appointments May fill in as a surrogate for a provider on leave Additional Information: The VA seeks to gain knowledge of potential qualified sources and their size classification (HUBZone, 8(a), small, small disadvantaged or service disabled veteran owned small business) relative to NAICS Code 561320, Temporary Help Services; size standard of $27.5 Million. Responses to this Sources Sought will be used by the Government to make appropriate acquisition decisions and developing a potential procurement strategy. After review of responses to this sources sought synopsis, a solicitation announcement may be published in the FedBizOpps website. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond directly to any corresponding solicitation by submitting a proposal. Responses: If you are interested and are capable of providing the required services please provide the requested contact information. The intended contract period will be for one base period (six months) and one option periods (six months).
