COMMUNITY-BASED, TRANSITIONAL HOUSING Description: In response to an increasing need for Community-Based, Transitional Housing, and a gradual reduction of on-campus VAGLAHS Transitional Housing: VAGLAHS requests a Contractor capable and responsible for providing ALL facilities/housing, staffing, supervision, and supplies/materials/provisions necessary for up to 80 single Veterans; homeless and suffering from mental illness, physical disabilities, and/or substance abuse problems. The VAGLAHS provides a broad range of housing and case management services to homeless Veterans to facilitate access to medical, mental health and rehabilitative services. The primary mission of VAGLAHS is to engage homeless Veterans in treatment services by placing VA-eligible Veterans in temporary, safe, and secure environments; supporting rehabilitation goals and reducing homelessness among our Veteran population. The contractors Community-Based, Transitional Housing facilities will preferably be within a 30 mile radius of the: Department of Veterans Affairs VAGLAHS 11301 Wilshire Blvd Los Angeles, CA 90073 General Supportive services such as Transportation for Veterans to and from VA medical and/or mental health appointments if needed or requested; Storage and monitoring of medications; Required social services in collaboration with VA staff and/or other community resources such as: assistance in obtaining a driver s license, obtaining a DD Form 214, or completing applications for public assistance/ services; Opportunities to participate in spiritual worship; Opportunities to participate in physical activities. Case Management services such as Individual counseling with a focus on areas including but not limited to: self-care skills, adaptive coping, education of the process of illness and recovery, social skills enhancement, and vocational rehabilitation as appropriate; Instruction in and assistance with health and personal hygiene; Scheduled visits with each Veteran resident to discuss progress towards goals established in the individual service plan (ISP); Referrals to supportive social services, in collaboration with VA staff and/or other community resources; Discharge planning after consultations with the Healthcare for Homeless Veterans (HCHV) staff, to appropriate follow-up housing; including assistance with referrals and applications. The Contractor shall be responsible for admitting and assessing Veterans including initial and ongoing assessments. The care-planning process shall include coordination, supervision, and evaluation of the care and services provided during the time of treatment and upon discharge. Every Veteran shall have an ISP that will be reviewed and discussed with Case Management staff. --END SOLICITATION NOTICE--
