- Attachment Two - Statement of Work, Solicitation Provisions & Contract Clauses Attachment One - Brand Name Justification (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, in conjunction with FAR 13.5, as applicable, and as supplemented with additional information included in this notice. This acquisition is being conducted in accordance with FAR Subpart 13.5 Simplified Procedures for Certain Commercial Items. Simplified acquisition procedures will be followed. (ii) This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. (iii) This combined synopsis/solicitation will result in the issuance of a firm fixed price commercial supply contract. The period of performance is from date of award through completion or through September 30, 2017. (iv) This combined synopsis/solicitation is being issued as a full and open competition. The NAICS Code is 325320 with a size standard of 1,000 employees. (v) This requirement is for the United States Department of Agriculture (USDA), Animal Plant Health and Inspection Service (APHIS), Plant Protection and Quarantine Services (PPQ) program. (vi) The objective of this procurement is for the purchase and transport of the pesticide chemical Dow AgroSciences Telone II (1, 3-dichloropropene 97.5 %, inert ingredients 2.5 %) for fumigation treatments in Bingham and Bonneville Counties in Idaho. This solicitation is for the transport, storage, and delivery of the chemical Telone II (1, 3-dichloropropene 97.5 %, inert ingredients 2.5 %). (vii) REQUIREMENT: This is a brand name request. See Attachment One entitled "Attachment One - Brand Name Justification and Approval" for this brand name request. (viii) The contractor shall provide a firm fixed price per gallon for the delivered product. Line item 001: Dow AgroSciences nematicide Telone II (1, 3-dichloropropene 97.5 %, inert ingredients 2.5 %) for a quantity of 24,000 gallons. The contractor shall provide a price per gallon. The selected contractor must be able to provide the entire quantity of Telone. According to the DOW AgroScience Representative Ryan Roslak, he stated that under the marketing letter in effect with Dow, Dow has agreed to supply the vendor with the product for this purchase and it would be separate from their current allocation. (viii) Shipping and delivery is FOB destination; therefore, delivery of the item must be included in the price for the line item as well as all overheard and material costs to deliver the items to the various fields in Bingham and Bonneville Counties in Idaho where the fumigation services will be performed. All the fields to be treated are within a 8 ½ mile radius. (ix) The Contractor will be required to deliver the product to the applicators equipment for each application. It is the Contractor's responsibility to work with the applicator to coordinate delivery of the product in accordance with the contractors schedule for application. The first delivery is estimated to be scheduled on or about August 15. Multiple deliveries will be required in order to complete the work. Work with the applicator will be scheduled so the work will be completed within 15 operational days from the start of services. The USDA APHIS will solicit for an application contractor separately and the Contractor will be provided the name and contact information of the application Contractor when the contract is issued to that vendor. (x) SPECIFICATIONS & STATEMENT OF WORK: See attachment two for the Specifications and Statement of Work. (xi) See Attachment Two for the Solicitation Provisions and Contract Clauses applicable to this solicitation and ensuing contract. (xii) EVALUATION OF QUOTES: The Government will award a contract in accordance with FAR part 13.106-1 and 13.106-2 resulting from this solicitation to the responsible contractor whose quote conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Award will be made to the lowest priced technically acceptable quote meeting or exceeding the technical evaluation factors listed in this Request for Quote/solicitation which are as follows: 1. Contractor has an Idaho pesticide dealer's license, with certification for selling and distributing the restricted use pesticide (Telone II), or the proven ability to obtain one before contract award. (The estimated award date of the contract is August 3.) 2. Contractor demonstrates the ability, has the product, labor services and ability to provide the chemical to the applicator as needed to ensure fumigation can be completed within 15 operational days from the start of fumigation services. 3. Contractor is a Dow AgroSciences Certified Telone Dealer. The Contractor must provide a copy of the Dow AgroSciences certification and Idaho pesticide dealers license with their quote. (xiii) Quotations are due to the Minneapolis Contracting Office by 11:00 AM Central Time, Monday, July 24, 2017. Please reference the solicitation number on your documents. Quotes shall be submitted via email to the assigned Contracting Officer Corinne Nygren at Corinne.m.nygren@aphis.usda.gov Ms. Nygren can be reached by phone at (612) 336-3235. (xiv) Quotes should be of sufficient detail to determine their adequacy. If a contractor's quote does not contain all of the items listed below, the quote may be considered incomplete for evaluation purposes and no further consideration will be given to the contractor's quote, thereby making the contractor ineligible for award. As a minimum, contractors must submit the following to the Contracting Officer to be considered and have a complete quote: 1) Price Quote - a price for line item 001. This page should also include the vendor's full name, address and DUNS#. 2) The required documentation requested above in section (xii) entitled EVALUATION OF QUOTES, which includes information that addresses the technical factors to verify your company's ability to provide the product and services described in the Specifications and Statement of Work in Attachment Two. 3) A completed copy of the provisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items OR confirmation that this has been completed on-line. The Online Reps and Certs Representations and Certifications can be completed at www.SAM.gov, or you may use the provision in Attachment Two.
