- AMENDMENT NOTICE: THIS SOLICITATION IS HEREBY EXTENDED AND WILL NOW CLOSE ON THURSDAY, JULY 13, 2017, 10:00 AM CENTRAL TIME. A CHANGE IS MADE ONLY TO PERIOD OF PERFORMANCE, FROM: BASE+2 OPTIONS TO: BASE+4 OPTIONS, REVISED FORMS ARE ATTACHED. NO OTHER CHANGES ARE MADE TO THE BELOW NOTICE. (1) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6 as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (2) The solicitation number is HSBP1017Q0088 and is issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-95. The associated North American Industrial Classification System (NAICS) code for this procurement is 541940 with a small business size standard of $7.5 million. (3) This procurement shall be set-aside for small business. All qualified, responsible sources may submit an offer that shall be considered by the agency. Offerors are advised that they are responsible for obtaining amendments. The resulting contract shall be awarded to the offeror who provides the lowest price technically acceptable. (4) The U.S. Department of Homeland Security (DHS), Customs and Border Protection, intends to award a Blanket Purchase Agreement (BPA) for the procurement of Veterinarian Services and canine supplies. It is required that all contractors submitting a quote review the attached STATEMENT OF WORK and all related documents, refer to Item #6 below. (5) Period of performance shall be Date of Award - 12 Months, plus one 12-month option. (6) ATTACHMENTS: a. RFQ: SF-18 Form - HSBP1017Q0088 b. FAR Clauses & Provisions c. Statement of Work d. Attachment A - Pricing Schedule (Excel Spreadsheet, see bottom tabs for each year) e. BPA Supplemental terms (to be completed only at time of contract award). (7) BID INSTRUCTIONS: All contractors must complete and submit the following (2) two documents via email by 5:00 PM (Central Time), Friday, June 30, 2017 to susan.gonzalez@dhs.gov: a. Page 1 of the RFQ SF-18, with completed blocks 13a - 13f, 14, 15, and 16a-16c. b. Attachment A - Pricing Schedule (Excel Spreadsheet-completed with bid pricing for Base and Option years) (8) EVALUATION FACTORS: Best Value/trade off: Award shall be made to the contractor whose offer represents the best value to the Government using the trade-off process source selection process. Technical capability, past performance and price shall all be considered, with technical capability being significantly more important than price.
