- SOURCES SOUGHT SYNOPSIS DESCRIPTION: This Sources Sought Synopsis shall be used to determine the availability of potential sources having the skills and capabilities necessary to perform/provide Integrated Pest Management (IPM) program for Sanitation, Structural Inspection, and Pest Control Services at Overton Brooks VA Medical Center (OBVAMC). All interested vendors are invited to provide information to contribute to this market survey/sources sought synopsis including, commercial market information. This is not a solicitation announcement. This is a sources sought synopsis only. Questions should be submitted by email to anthony.mitchell4@va.gov. Provide only the requested information below. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (Service Disabled/Veteran Owned Small Business (SDVOSB/VOSB), Hubzone, 8(a), small, small disadvantage, woman owned small business, or large business) relative to NAICS 561710, Exterminating and Pest Control Services (size standard of $11 million). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published on the FBO website. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors must respond to the solicitation announcement in addition to responding to this sources sought announcement. Please demonstrate the contractor's ability to perform services of this nature as the course of its daily business operations. Services shall be performed in accordance with industry standards and practices by qualified personnel. Responses must be emailed to anthony.mitchell4@va.gov no later than NOON CST, July 18, 2017. Your response should include both the STATEMENT OF CABABILITY and BUSINESS SIZE AND SOCIO-ECONOMIC STATUS information as explained below. Please place Attention: Integrated Pest Management (IPM) OBVAMC, in the subject line of your email. This notice is to assist the VA in determining sources only. A solicitation is not currently available; therefore, DO NOT REQUEST A COPY OF A SOLICITATION THAT DOES NOT EXISTS. If a solicitation is issued it will be announced at a later date, and all interested parties shall respond to that solicitation announcement separately from the responses to this announcement. REQUESTED INFORMATION: (1) STATEMENT OF CABABILITY: (1) Submit a brief [five (5) pages or less] capability statement explaining your capability to perform commercial Integrated Pest Management (IPM) program for Sanitation, Structural Inspection, and Pest Control Services at Overton Brooks VA Medical Center. Include the number of qualified personnel need to accomplish this service. Include experience in performing these services for the VA, other Government (Federal or State) agency, or for a private medical facility. Please specify your maximum capacity and your availability to start. SPECIFIED REQUIREMENT: The contractor shall communicate its capabilities to accomplish the following: (See next page) PERFORMANCE WORK STATEMENT Description of Services/General Information. The contractor shall provide all management, licenses/certifications/insurance, laboratory testing, tools, supplies, equipment, transportation, supervision and labor to develop and implement an Integrated Pest Management (IPM) program for Sanitation, Structural Inspection, and Pest Control Services at OVERTON BROOKS VA MEDICAL CENTER. Service shall be performed in a manner that will ensure the health and general well-being of hospital staff, medical staff, patients, visitors and their families, and effectively control pests. Pest control services shall be performed in accordance with (IAW) commercial and Department of Veterans Affairs (DAV). All pest control service requests shall be submitted via contractor supplied toll free telephone number or government approved electronic communication. The Contractor will report to the Contracting Officer's Representative (COR) or designee upon arrival and before departure from the installation. Overton Brooks VAMC, 510 E. Stoner Ave. Shreveport, LA 71101. Contractor will be on-site weekly every Thursday and provide a comprehensive plan to ensure all areas of the facility are routinely checked and appropriately treated. A detailed service report/invoice shall be submitted to the COR. All buildings to be inspected located on the campus will be serviced to include, but not be limited to, the following: Bldgs. 1,2,3,4,6,12,14,16,33,42,55, compactor area, recycling area (13 total) Sanitation and Structural The main hospital facility shall be inspected and serviced weekly. In addition, all other services listed below shall be serviced monthly. A detailed service report/invoice shall be submitted to the COR. Nutrition and Food Service, 510 E. Stoner Ave. Shreveport, LA 71101 (basement). Contractor will be on site monthly, (except federal holidays) and provide a comprehensive plan to ensure all areas of the department are routinely checked and appropriately treated. Canteen Service, 510 E. Stoner Ave. Shreveport, LA 71101 (2nd floor). Contractor will be on site monthly, and provide a comprehensive plan to ensure all areas of the department are routinely checked and appropriately treated. Sterile Processing Department (SPD), 510 E. Stoner Ave. Shreveport, LA 71101 (basement). Contractor will be on site monthly, and provide a comprehensive plan to ensure all areas of the department are routinely checked and appropriately treated. INTEGRATED PEST MANAGEMENT (IPM) PLAN. The contractor shall develop and submit an IPM plan within 30 days of contract start date, and update the plan within 10 days following any changes to chemicals or strategies are made for approval. IPM plan shall be a coordinated use of pest and environmental information and pest control methods to prevent unacceptable levels of pesticides, and with the least possible hazard to people, property and then environment. Approved compounds for pest control shall be: 1. Boric acid 2. Disodium orthoborate tetrahydrate 3. Silica gels 4. Diatomaceous earth 5. Non-volatile insect and rodent bait in tamper-resistant containers 6. For crack and crevice treatment only microbe-based insecticides, pesticides made with essential oils (not including synthetic pyrethroids) without toxic synergists, and materials for which the inert ingredients are non-toxic and disclosed. Materials proposed for pest control shall not be determined by the US EPA (Environment Protection Agency) as a probable, likely, or known: Carcinogen Endocrine disruptor Mutagen Reproductive toxin Developmental neurotoxin Immune system toxin Toxicity I or II pesticide In the organophosphate or carbamate, chemical family Contains inert ingredients categorized as List 1: Inert of Toxicological Concern. Concern Pest control steps: Record all activities on a facility-specific log sheet. Infestation from outside: Pest Management begins outside the building. Therefore, survey the exterior of the building for cracks, crevices, holes in external walls, and poor seals at doors and windows. Report all suspected areas to the maintenance department for correction. When a pest problem is identified baits and traps shall be the first line of defense. Traps for both insects and rodents should be carefully and strategically placed and monitored. When necessary, a pesticide product specific to the species being eliminated shall be used. The building and site shall be regularly inspected for pests at a minimum as required herein. Any pests observed shall be monitored and the need for pesticides shall be evaluated and documented. Provide recommendations to reduce likelihood for infestation, including sanitation, structural repairs, mechanical controls, or living biological controls, to provide alternatives to chemical options. The above compounds can be applied without giving notice to building occupants. In an emergency, as defined below, the following protocol shall be followed: See Section 1.3 Definition of an Emergency: A sudden large increase in a previously controlled pest population A sudden infestation of pests that poses significant threat to occupant health and safety or potential to cause serious property damage A sudden infestation of pests that causes an area of the site/building to be unusable In addition, the IPM plan shall comply with all applicable Environmental Protection Agency (EPA), Occupational Safety and Health Administration (OSHA), federal and state/local regulatory guidance on pesticide usage, disposal and applications. The contractor s IPM plan shall establish the strategy and methods for conducting a safe, effective, and environmentally sound pest management program. In developing the IPM plan, the contractor shall address continuous monitoring, pest response and removal procedures, record keeping, warranties, pest education and communication to prevent pests and disease vectors. The IPM approach shall use targeted (effective and environmentally sound) methods including habitat/facility modification, biological/genetic/cultural control, mechanical/physical control, and where necessary with the judicious use of the least hazardous pesticides using all Federal, State, County and local requirements. A list of all pesticides to be used for pest control services shall be included in the IPM plan. The contractor shall only use pesticides from Federal listings approved by the Contracting Officer s Representative (COR). The contractor shall supply electronic copies of Material Safety Data Sheets (MSDS) and Chemical Labels to the COR within 10 days after contract award and 30 days prior to the start of each contract year. The IPM plan requires approval by the COR prior to implementation. The Contractor shall review and update the plan annually and as strategies or chemicals change for all pest control categories. The updated IPM plan shall be submitted to the COR within 10 days after contract award and 30 days prior to the start of each contract year. 1.2. BASIC SERVICES 1.2.1. Common Pests. Common pests are described as any insect, invertebrate, arachnid, rodent, mammal, bird, or snake that causes an economic loss, or presents a hazard. These pests include mice, rats, squirrels, gophers, birds, bats, ants, bees, snakes, mosquitoes, gnats, beetles, weeds, termites, cockroaches, wasps, spiders and other crawling and flying insects within and around the facilities identified herein and to any other future requirements. Treatment should also include for the prevention of reappearance of all pests. This list is not all inclusive and shall not be a complete list of the pests the contractor shall be responsible to control. Contractor shall ensure applicators are trained in the proper and safe handling of snakes. 1.2.2. Structural Pest Control Services 1.2.2.1. Monthly Inspections and Treatment. Inspections of all facilities shall be accomplished monthly. All inspections and treatments performed within the month shall be included in the monthly service charge. The contractor shall complete all requests for treatment/service within two (2) hours of notification, or coordination with the COR or designee. Treatments shall be performed at a time approved by the COR or designee. The contractor shall coordinate all treatments performed with the COR or designee. The contractor shall provide services in accordance with the requirements defined for each location. Any changes to the schedule will be coordinated with the COR or designee. Inspections and treatments will be completed in all areas and locations covered by the contract. 1.2.2.2. Areas of Treatment. Contractor is responsible for treating all areas of the facilities to include drains, elevator wells, interstitial areas, basements, storage rooms, and all areas of the grounds on each campus. All pest control treatments should be carried out in accordance with the schedule approved by both parties and Contractor shall ensure least possible disruption to day to day operations while providing the Services at OBVAMC facilities. Contractor will coordinate in advance with the COR or designee when any type of major service is necessary that causes the disruption of normal operations. 1.2.2.3. Pest Control for Facilities. The contractor shall maintain a log book on site at each location serviced. The log book will be used to identify issues and concerns identified at each location. Contractor will complete all routine pest control requests identified during each visit per the established schedule. Contractor shall coordinate with the COR or designee on pest management practices to keep pests under control using the least amount of pesticides. The contractor shall contact the COR or designee prior to any pest control actions. 1.2.3 Mosquito and Larvae Control. Contractor shall perform treatment and ensure effectiveness of treatment of areas that have been identified as mosquito harborage and breeding areas by the COR. All treatments to identified areas shall be accurately documented and reports will be provided to the COR upon request. 1.3. EMERGENCY SERVICES. The Contractor must be able to respond at any time and provide 24 hour emergency call out services at no extra charge to the government where treatment to the facilities has been carried out by Contractor in accordance with the schedule but the facility is still not free of the pest. In such cases Contractor, shall provide appropriate Pest Control services as needed to rectify identified pest issues as identified by the government. Contractor must ensure that a representative is on-site within 2 hours after receiving an emergency notification from the government. Note: Contractor may have to perform work at times other than the normal duty hours for emergencies. 1.4. PESTICIDE MIXING AND STORAGE. No pesticides will be stored at any government location. All unused pesticides, empty pesticide containers and residue shall be disposed of properly at an approved disposal area. In the event the contractor spills or releases any hazardous substances, the contractor shall immediately notify the COR. Chemical mixing for immediate application may be accomplished at the site of application/treatment and only state certified applicators may mix or apply pesticides. 1.5. SAFETY. Contractor shall wear all safety items while mixing and applying chemicals. When the label states that an eye irritation may occur, or lists flushing eyes, face and eye protection shall be worn as part of the required safety items. Contractor shall provide a spill container at mixing areas to ensure that no chemicals impact an area that is not being treated. The contractor shall have an operational emergency eyewash kit available at each mixing location. 1.6. FACILITY MANAGER/HOUSING OCCUPANT TRAINING. Contractor shall provide training to government staff on methods to reduce and prevent pest infestations. This training shall be accomplished semiannually through newspaper articles, flyers, pamphlets, and briefing slides submitted to the COR for approval prior to use. Newspaper articles, flyers, pamphlets, and briefing slides shall vary based on the season and the types of pests that are common during that period. 1.7. PEST CONTROL RECORDS AND REPORTS. 1.7.1. The contractor shall maintain a complete pest control record and reports that will be provided within 24 hours after receipt of a request for information from the COR. This record provides all information required by the Department of Veterans Affairs, Federal, State and Local Regulations. All reports shall have the Percentage of Active Ingredient used and for each chemical used. Contractor will provide diagram of facility where bait traps are located. 1.7.1. 1.Weekly Report. The contractor shall provide a written report following each visit that will be maintained in the station log book. The report shall include service call request number, facility number, service address, date and time requested, date and time completed, the problem reported, requester name and telephone number. (required) 1.7.1.2. Monthly Report. Monthly reports shall be electronically submitted to the COR within the first five days of the following month. (optional) 1.7.1.3. Quarterly Report. Quarterly reports are to be electronically submitted to the COR within the first five days of the month following each quarter. (optional) 1.7.1.4. Annual Report. Annual reports are to be turned in to the COR by the fifth of October each year. (Required) 1.7.2. Pest Control Monthly Summary. The contractor shall provide the COR with a monthly summary of service calls. The Service Call Summary shall be provided electronically within the first five days of the month. The monthly Pest Control summary shall list quantities of calls to each location. The contractor will maintain a log book at each location that the contractor will maintain. Log book will be available for the government to document any pest control issues that arise and the Contractor will be responsible for documenting all services performed. The Contractor will provide statistical data as required by the government. This includes types and quantities of chemicals, pesticides, and any other materials used in the performance of pest control services, types of treatments provided over a given period (monthly, quarterly, annually), and types/quantities of pests that were treated. This information will be provided within one work day of request. 1.8. CONTRACTOR EMPLOYEES. The contractor shall have a State of Louisiana certified pest control manager who is license, certified, and insured specifically assigned to support this contract available within two hours to meet with government personnel (telephonically or in person) during duty hours. All contractor employees shall comply with State of Louisiana Code of Regulations. All contract employees that apply or mix pesticides shall have a current State of Louisiana Pesticide Applicator Certificate. The contractor shall be responsible to ensure that the licenses certification, insurance and training of contractor personnel are kept current and copies provided to the COR. The contractor must provide a certificate of training for bedbugs annually. 1.9. HOURS OF OPERATION. The contractor shall provide sufficient personnel to cover all required services on Thursdays during normal hours of operation which are 8:00 am to 4:30 pm, weekly. Emergency Services may require work on weekdays, holidays, Saturday and/or Sunday (see paragraph 1.3). QUALITY CONTROL PLAN. The contractor shall develop and maintain a quality control plan (QCP) to ensure systematic inspection of facilities for pests, a customer service call/emergency response system for pest removal, and a continuous pest prevention education program for government staff. The contractor shall develop and implement procedures to identify and prevent defective services from recurring on all services required by this PWS. As a minimum, the contractor shall develop quality control procedures that address the areas identified in Section 2, Services Summary. The contractor s QCP shall be written and submitted to the COR within 10 calendar days from the date of award and within 30 calendar days prior to the start of each option year or as changes occur. The Contractor shall identify their quality control representative in writing to the government COR for notification and resolution of valid customer complaints or Quality Control issues. QUALITY ASSURANCE. The government will evaluate the contractor s performance by appointing COR to monitor performance to ensure services are received. The COR will evaluate the contractor s performance through intermittent on-site inspections of the contractor's quality control program and receipt of complaints from hospital personnel. The government may inspect periodic tasks as completed or increase the number of inspections if deemed appropriate because of repeated failures discovered during inspections or because of repeated customer complaints. Likewise, the government may decrease the number of inspections if performance dictates. The government will also receive and investigate complaints from various customers located throughout the installation. All customer complaints will be submitted to the COR. However, the COR shall make final determination of the validity of customer complaint(s) in cases of disagreement. QUARTERLY PERFORMANCE MEETINGS. The Contractor shall be available to meet periodically with the COR and Contracting O, other government personnel as appropriate, to discuss the Contractor s performance. The following issues shall be discussed: opportunities to improve the contract; any modifications required of the contract; unsatisfactory inspections and trends against each performance objective observed; positive performance; and steps taken by the Contractor to prevent unsatisfactory occurrences in the future. The Contractor shall provide a summation of unsatisfactory inspections and customer complaints and provide insight into any identified trends. The minutes of these meetings will be signed by the CO and any other signatures as deemed appropriate, distributed to the functional area and the contractor. Should the contractor not concur with the minutes, the contractor shall provide written notification, within 10 business days, to the CO identifying areas of non-concurrence. CONTRACTING OFFICER'S REPRESENTITIVE. Contract Monitoring Procedures: At the time of contract award, the Contracting Officer will appoint a Contracting Officer s Representative (COR) to assist with the contract monitoring requirements. The COR will monitor such items as quality of service, timeliness of performance, customer service, cost control, and business relations supporting documentation. In accordance with FAR 1.601 (a) Contracts may be entered into and signed on behalf of the Government only by contracting officers. A COR may not modify the stated terms of the contract, including extending the length of the contract period. The contractor shall sign and return the COR Delegation Letter to acknowledge receipt of the COR Delegation Letter.
