Description

This is a COMBINED SYNOPSIS/SOLICITATION NOTICE for Dental Laboratory Casework Responses to this Combined Synopsis/Solicitation must be in writing and submitted on the attached Request for Quotation. This acquisition implements a 100% set aside for Service Disabled Veteran Owned Small Businesses (SDVOSBs). The NAICS Code is 337127; size standard is 500 employees. Responses to this notice shall include the following: (a) company name    b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code   (g) Tax ID Number (h) and must    provide a capability statement that addresses the organizations qualifications and ability  to perform as a contractor and provide the commodities described within the Request For Quotation.         Dental Laboratory Cabinetry/Casework See Statement of Work below. The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this Combined Synopsis/Solicitation. This notice shall not be construed as a commitment by the Government to award a contract, nor does it restrict the Government to a particular acquisition approach. Notice to Potential Offerors: All Offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM located on the web at www.sam.gov). It is desirable that any Offeror to have completed their business Online Representations and Certifications Application (ORCA). Additionally, all Service Disabled Veteran Owned Small Businesses who respond to a solicitation on this project must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov.  Requests for information and resultant responses shall be submitted directly to the contracting officer, Paul Speedling at Paul.Speedling@va.gov. STATEMENT OF WORK BACKGROUND Building 3 at Montrose is being renovated to accommodate a new Dental Suite. VA is to supply cabinetry/casework for the Laboratory. A floor plan and specifications have been developed to maximize efficiency and meet all functional requirements provided by Dental Service. OBJECTIVES To supply specialized cabinetry to activate Dental Laboratory in Building 3 Montrose, NY. PERIOD OF PERFORMANCE Once ordered, the material has a lead time of 10-12 weeks. PLACE OF PERFORMANCE Montrose, NY Building 3 Dental Laboratory PRODUCT REQUIREMENTS/SPECIFICATIONS: Please refer to attached item list for specifications and measurements. This plan was developed to VA s specifications, and accurately diagrams the product sought for procurement. 1. Standard base cabinets are at 35 5/8 height with no top. 2. Counter top material: ½ Solid Surface in Sanded Vermillion or Sanded Gray ; 1 1/2" Thick Stainless Steel; 1 ¼ Soap Stone. Cabinetry and countertop finishes to be selected from Manufacturer s offerings. Freight to be Included F.O.B. Destination All bids will be required to provide a detailed floor plan with cabinetry specifications. All items to be delivered to and inspected at Montrose Warehouse.
