Description
THIS REQUEST FOR INFORMATION (RFI) IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. THE SUBMISSION OF PRICING, CAPABILITIES FOR PLANNING PURPOSES, AND OTHER MARKET INFORMATION IS HIGHLY ENCOURAGED AND ALLOWED UNDER THIS RFI IN ACCORDANCE WITH (IAW) FAR 15.201(e). DISCLAIMER This RFI is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. IAW FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification relative to NAICS 337215 (size standard of 500 Employees). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published. Responses to this sources sought synopsis are not considered adequate responses for a solicitation announcement. The Department of Veterans Affairs (VA), VISN 22 Network Contracting Office, is seeking sources that can provide Mobile System and Shelving that at a minimum meet the following salient characteristics with a brand name or equal product for the VA Los Angeles Healthcare System: ITEM NUMBER DESCRIPTION OF SUPPLIES/SERVICES QUANTITY UNIT UNIT PRICE AMOUNT 0001 Aurora Mobile System and Shelving -Two mobile carriages at 36"W x 144 ?"L x 82 7/8" overall height each with 6 sections of back to back shelving at 48" wide x 18"d each with two bin dividers per level and 6"bin fronts. -Static shelving: 5 sections - 42" W and 48"W x 18"D x 85 ?"H with 4 openings each with and 2 bin dividers each level and 6" bin fronts. Paint finish: TBD 1 EA 0002 Aurora Mobile System and Shelving One stationary platform - 36"W x 72 ?"L x 81-7/8" overall height, with two sections of open shelving with three openings. -One mobile carriage - 36"W x 72 ?"L x 81 7/8" overall height, with two rows of back to back shelving at 36"W x 18"D. Shelving has 5 openings with two bin dividers per level and 6"H bin fronts. -Two mobile carriages - 36"W x 48 ?"L x 81 7/8" overall height, each with two sections of back to back shelving at 48"W x 18"D with 5 openings with two bin dividers per level and 6"H bin fronts. -Static shelving: Two sections-t 30"W x 18"D x 76 ?"H each with 5 openings with one divider per level and 6" bin fronts. Paint finish: TBD 1 EA 0003 Aurora Mobile System and Shelving -Two sections - 36" W x 18" D x 85 ?" H with 5 shelves -One section - 36" W x 24" D x 85 ?" H with 5 shelves -Two sections - 42" W x 24" D x 85 ?" H with 5 shelves -One section - 48" W x 24" D x 76 ?" H with 5 shelves Paint finish: TBD 1 EA 0004 Installation: To be completed in one continuous phase. All product delivered in one trip, unless otherwise stated. All work to be done during regular business hours. Any delays due to construction or other trades will have additional charges. Items that require to be stair carried, will be at an additional charge. 1 EA ******OR EQUAL TO ITEMS LISTED ABOVE***** The information identified above is intended to be descriptive, not restrictive and to indicate the quality of the supplies/services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the supplies/services that fulfill the required specifications. If you are interested, and are capable of providing the sought out supplies/services, please provide the requested information as well as the information indicated below. Response to this notice should include company name, address, point of contact, size of business pursuant to the following questions: (1) Is your business a: Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), Hubzone, Woman Owned Small Business (WOSB), Large Business, etc.? (2) Is your company considered small under the NAICS code identified under this RFI? (3) Are you the manufacturer or a distributor of the items being researched above? (4) If you re a small business and you are an authorized distributor/reseller for the items identified above, do you alter; assemble; modify; the items requested in any way? (5) If you do, state how and what is altered; assembled; modified? (6) Does your company have FSS GSA/NASA SEWP/NAC or any other federal contract? If so, please provide the contract number. (7) If you are an FSS GSA/NASA SEWP/NAC or federal contract holder, are the items you are providing information for available on your schedule/contract? (8) Please submit your capabilities in regards to the salient characteristics being provided and any information pertaining to equal to items to establish capabilities for planning purposes? *** Submissions addressing Section (8) should show clear, compelling and convincing*** evidence that all equal to items" meet all the salient characteristics. Responses to this notice shall be submitted via email to Carlos.Magana@va.gov. Telephone responses shall not be accepted. Responses must be received no later than 1:00 p.m. July 19, 2017 PST. If a solicitation is issued it shall be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this sources sought. Responses to this sources sought notice are not a request to be added to a prospective bidders list or to receive a copy of the solicitation. Please reference VA262-17-N-1397 in all correspondence. Statement of Work This project is part of the Department of Veterans Affairs Greater Los Angeles Veterans Health Care System Located at: 11301 Wilshire Blvd., Los Angeles, CA 90073 1. INTRODUCTION This statement of work (SOW) describes the requirement for the Filing Cabinet Replacement project at the VHAGLAHCS. This is to provide new centralized filing, shelving and storing of patient records. 2. SCOPE OF WORK: This project consists primarily of condensed filling storage to be delivered at the above address. Final approval from Designer required before placing orders. Do not use VA dumpsters for disposal of scrap/waste without prior VA approval. 2.1. OVERVIEW The GLA and CBOC areas is undergoing a refurbishment project due safety and privacy issues are scheduled for replacement to meet requirements. Delivery is to occur between the hours of 8:30 am and 12:00pm, during normal working hours. 2.2.1. QUANTITIES The quantities of each item are listed as noted below. Awarded vendor shall include purchase order number on all shipping labels to include contents. Qty Item Description 3 ~ Aurora Mobile System and Shelving for Storage Rooms #3005, 3025B & 3025H 2.2.2. SUBSTITUTION REQUESTS No substitution requests will be accepted. All bids must include exact specifications as indicated in section 2.2.1 Sections. For equal to product consideration the following information must be provided: (1) Meet the salient physical, functional, or performance characteristic specified in this RFI; (2) Clearly identify the item by (i) Brand name, if any; and (ii) Make or model number; (3) Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and (4) Clearly describe any modification the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modification. 2.3 WARRANTY All workmanship and furniture quality shall be covered under warranty by the selected vendor for lifetime (limited). 2.4 VENDOR FURNISHED ITEMS AND RESPONSIBILITIES The selected vendor shall furnish all items as described in the statement of work. 2.5 DELIVERY 2.5.1. DELIVERY and INSTALLATION GLA Interior Designer will coordinate the delivery date and time with the selected vendor. 3. EVALUATION OF PROPOSALS Proposals shall be evaluated based on in order of importance Price, Quality, Time of Delivery. 4. INSPECTION AND ACCEPTANCE The Interior Designer shall inspect the delivery and sign off of any and all missing or damaged products in the form of a delivery manifest, vendor provided. The Interior Designer shall ensure all delivery of merchandise is completed and received satisfactorily prior to acceptance. Disputes shall be resolved by the Contracting Officer. 5. DELIVERY/STORAGE REQUIREMENTS Deliver materials to job in manufacturer's original sealed containers with brand name marked thereon. Package to prevent damage or deterioration during shipment, handling, storage and installation. Store products in dry condition inside enclosed facilities. All prices quoted at the placement and acceptance of order shall be firm fixed price. All delayed delivery arrangements, within 60 days, shall be at no additional cost to the Government. 6. Interference to Normal Function: The selected vendor may be required to interrupt their work at any time so as not to interfere with the normal functioning of the facility, including utility services, fire protection systems, and passage of facility patients, personnel, equipment and carts. In the event of an emergency, work performance may be stopped and rescheduled at no additional cost to the government. The selected vendor s personnel shall inform the Interior Designer or the designee of the need to gain access to secured areas. If access is required to secure areas, prearranged scheduling will be made with Interior Designer or designee.
