- Statement of Work for the NDI Assessment Requirement Clauses Applicable to Synopsis/Solicitation for NDI Assessment Requirement This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number for this requirement is 2017-Q-12784 and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. QUOTERS ARE ADVISED AN AWARD MAY BE MADE WITHOUT DISCUSSION OR ANY CONTACT CONCERNING THE QUOTES RECEIVED. Quoters should not assume they will be contacted, or afforded the opportunity to qualify, discuss, or revise their proposals. Therefore, the initial quote should contain the quoter's best terms from a technical and price standpoint. All quotes must include the full name, address, and DUNS number of the business to be named on the purchase order as well as the full name, phone number, and email address of the person who can accept the final purchase order. It is the quoter's responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions may be accessed via the Internet at website http://farsite.hill.af.mil. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-95 (January 19, 2017) This is a full and open competition under North American Industry Classification System (NAICS) code 541690-Other Scientific and Technical Consulting Services with a small business size standard of $15 million. Quotes are due on Wednesday, July 26, 2017, by 3:00 PM Eastern Standard Time and must be sent via email to the attention of Eric Sullivan at lrv4@cdc.gov. Hard copies will not be accepted. The Offeror must cite RFQ Number 2017-Q-12784 in the email subject line. Evaluation and award of the purchase will be performed in accordance with applicable sections of FAR Part 12 Commercial Items and FAR Part FAR 13 Simplified Acquisition Procedures. The Offeror must have an active registration with the System for Account Management (SAM) prior to award of the purchase order in accordance with FAR Clause 52.204-7. The government intends to issue a firm, fixed price purchase order in accordance with FAR Part 13. Proposals must be submitted in two parts: a technical proposal and a pricing proposal. The Centers for Disease Control and Prevention, National Center for Health Statistics (NCHS), Division of Vital Statistics (DVS) has a need to evaluate and enhance the current National Death Index system. The National Death Index (NDI) is a repository of all deaths occurring in the United States (U.S.) which is used to provide scientific researchers with the means to determine whether individuals in their research study had died, and if so, the cause of death. The NDI is the only national U.S. data source containing information on both the fact and cause of death for all deaths occurring within the U.S. While NCHS operates/manages the NDI, the records are owned by the vital records jurisdictions (i.e., the 50 states, New York City, Washington DC, and the 5 US territories of Puerto Rico, Virgin Islands, Commonwealth of the Marianna Islands, Guam, and the American Samoa) which establish the overall rules for how data are to be accessed, distributed, and used. The purpose of this procurement is to work with the vital statistics jurisdictions to: Identify the technical, administrative, legal, policy, operational, and administrative barriers that limit access and use of the NDI; Develop strategies that may enhance access and utility of NDI for patient-centered outcomes research while ensuring the privacy, confidentiality, and ownership of the records and data; and Actively participate in a national workshop of Federal, selected jurisdictions, and researchers in developing a consensus strategy for the long-term improvement of NDI. In order to meet this objective, the Contractor must work closely with key vital statistics jurisdictions to ensure that their collective thoughts, wisdom, and ideas are documented and expressed at the national workshop and the national strategy. The Centers for Disease Control and Prevention intends to award a firm-fixed-price contract for this requirement, please provide a pricing proposal containing the below information including a list of line item number(s) and items, quantities, units of measure, and options, if applicable: Item Supplies/Services Unit Total Price CLIN 0001 Assessment of the NDI in accordance with the attached Statement of Work 1 Job $_______________ Your quote shall include the following: · Total Price for CLIN 0001 · Capabilities in accordance with the Statement of Work and Evaluation Factors Please see the attached Statement of Work (SOW) for a more complete description of the specific tasks to be completed under this contract. After evaluation of the following technical factors, award will be made to the lowest priced technically acceptable offer. Each offer will be evaluated on a pass/fail basis, so offerors are cautioned that they should submit responses in sufficient detail to enable the government to evaluate their capabilities and experience. Evaluation Criteria: 1) Demonstrated knowledge/expertise with the federal/state National Death Index program and the policies/statutes/laws governing the program; 2) Demonstrated experience in reviewing and processing research proposals. Knowledge and demonstrated experience with Institutional Review Boards and Human Subjects review protocols at the state and federal levels; 3) Demonstrated collaborations/partnerships with the state vital records programs and experience working with them to improve the mortality data system. To be eligible, the Contractor must be a “voice” for the collective state vital statistics programs and demonstrate this requirement through the provision of at least 10 reference letters from the vital records state registrars indicating that the Contractor can speak on their behalf; 4) Documented experience of being able to get state vital records programs to travel and participate in national meetings about some aspect of their program; 5) Experience in developing 508-compliant reports to federal agencies; 6) Experience in negotiating and implementing data use agreements between state and federal agencies; 7) Documented history of working with state vital records registrars, including assisting them in shaping policy/statutes related to death information.
