This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is: VA259-17-Q-0717, and it is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-85B. This solicitation is set-aside for Small Businesses. Date and place of delivery (FOB Destination): 51 Yale Building 5110 S. Yale Avenue Tulsa, OK 74135-7401 Delivery shall be within 30 days ARO. The clinic will be located on the second floor of an office building with two elevators. The provision at 52.212-1, Instructions to Offerors Commercial, applies to this acquisition. The provision at 52.212-2, Evaluation Commercial Items, does not apply to this acquisition. Offers will be evaluated LPTA. Offerors are advised to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications Commercial Items, with the offer. The clause at 52.212-4, Contract Terms and Conditions Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items, applies to this acquisition. Name and telephone number of the individual to contact for information regarding the solicitation: Shelby Hamilton Contract Specialist Department of Veterans Affairs, NCO-19 303-202-8415 shelby.hamilton2@va.gov STATEMENT OF WORK Tulsa Dental Furnishings Contractor shall be responsible for all aspects of furniture installation, service, reconfiguration, cleaning. Moving and the inventory control of excess furniture. Contractor shall have a minimum of three years of knowledge and experience with Herman Miller, Herman Miller Milcare, Steelcase 9000, Steelcase Wood, Teknion, Kimball Cetra, Kimball Wood, Artopex, Midmark Hospital Exam Tables, chairs and cabinetry. Contractor shall have a minimum of three years of experience in a Hospital setting and daily operations regarding code situations and patients. Contractor shall have hospital security clearance and is required to wear a hospital ID badge at all times. Contractor is required to work with the VA Contracting Officers Representative (COR) of his/her designee. General: The contractor will coordinate install locations with the VA. All variances to specifications shall be clearly defined in proposals. All items shall be specified from GSA sources. It is advantageous that the amount of different sources be limited in order to simplify procurement, to limit quantity of PO s required. Clearly define any non-GSA proposed items. All trim, accessories, and hardware necessary for a complete installation shall be included in the bid. Utilize the attached excel spreadsheet template to provide a quote for this project. Maximize use of quick ship items. All furnishings will be delivered and installed at the following location: 51 Yale Building 5110 S. Yale Avenue Tulsa, OK 74135-7401 Desks (D02): All case goods within offices shall be provided by a single manufacturer in order to match finishes. Workstations shall include box/box/file pedestals which shall be attached to workstations. Private office work surfaces shall be a plastic laminate wood grained surface. Filing/Storage, Lockers & Bookcases (F01,F02, L01, B01): F01 & F02, Storage units shall match workstations in finish and lock. Two keys per lock shall be provided. L01, Lockers shall be stacking and composed of steel. B01, Bookcases shall match workstations in finish Seating (S01, S03) S01 and S02 Guest Chairs and Bariatric Benches shall have an upholstered seat and back, arms, built in clean-out spaces and waterfall front. Upholstery shall be vinyl with a minimum abrasion resistance of 75,000 double rubs when tested with Wyzenbeek method. Glides shall be designed for either carpeted or resilient surfaces. Glides shall not mar or discolor resilient flooring surfaces. Wood finishes shall match workstations and sofa if applicable. S03, Desk Chair shall have vinyl back and vinyl seat. Chairs shall have a 5-star base and be rated for intensive use, be a medium size, with urethane adjustable arms (height and width adjustable), adjustable lumbar support, and adjustable seat pan. Upholstery shall be black vinyl with a minimum abrasion resistance of 75,000 double rubs when tested with Wyzenbeek method. Casters shall be designed for use on hard floors. Tables (T01, T02) T01, End tables shall match finish of guest chairs if applicable. T02, Conference Tables shall have a laminate top with grommets and base design. T07, Round tables shall match wood finish of guest chairs if applicable. Locks and Keying: All desks and storage components within a single workstation shall be keyed alike. Central file storage shall be keyed alike but shall be different from workstation locks. Two keys shall be provided per workstation when keyed alike and three master keys shall be provided. Keys and locks shall be numbered for ease of replacement and locks shall be clearly labeled with a key number except for those manufacturers who have removal format keys. Finishes: Finishes and upholsteries shall be selected by VA design staff. Where possible all metal trim of furnishings shall be brushed aluminum. Submittal Requirements: Installation cost shall be a line item and shall include price for receiving product (only product provided by the vendor and not the VA provided products), delivery to site for those items, and installation costs. In addition, please include monthly estimated storage fees applicable if the scheduled project completion were to change. Provide illustrations of all furniture proposed along clearly designating which literature applies to which item code. Provide a list of all variances in proposed product to the provided specifications. Provide all finishes and fabrics in grade specified for selection by VA Interior Design Staff. Fabric/finish selections should be standard product. Provide lead times for furniture specified from date order is received. Provide estimated duration and phasing of furniture installation.
