Description
The Department of Veteran Affairs, Dorn VA Medical Center, 6439 Garners Ferry Road, Columbia, SC 29209 has a requirement to provide all labor, supplies, tools, materials, transportation, parts and equipment necessary to remove and dispose of miscellaneous trash, garbage, yard debris and construction material at the William Jennings Bryan (WJB) Dorn, Veterans Affairs Medical Center, 6439 Garners Ferry Road, Columbia, South Carolina 29209. The Government expects to award to a single contractor a contract which shall be Firm Fixed Price with a firm quantity. The requirement contains a base year and 4 option years for a total of 5 years. This requirement is being set-aside for Service Disabled Veteran Owned Small Business participation only. NAICS code for this requirement is 562219 with a small business size of 500 employees. All responsible sources may submit a proposal, which shall be considered. Please see solicitation for additional information regarding this requirement Performance Based Statement of Work Solid Waste / Trash Removal 1. GENERAL The contractor shall provide all labor, supplies, tools, materials, transportation, parts and equipment necessary to remove and dispose of miscellaneous trash, garbage, yard debris and construction material at the William Jennings Bryan (WJB) Dorn, Veterans Affairs Medical Center, 6439 Garners Ferry Road, Columbia, South Carolina 29209. 2. BACKGROUND: The WJB Dorn Medical Center is a 216-bed facility, encompassing acute medical, surgical, psychiatric, and long-term care. The hospital is located in Columbia, South Carolina and provides primary, secondary, and some tertiary care. Annually, the medical center serves approximately 80,000 patients. Satellite outpatient clinics are located in Anderson, Greenville, Florence, Orangeburg, Rock Hill, Sumter, and Spartanburg, South Carolina. 3. PERIOD OF PERFORMANCE: One Base year with the possibility of four one year option periods. Base Year: 1 October 2017 30 September 2018 1st Option Period 1 October 2018 30 September 2019 2nd Option Period 1 October 2019 30 September 2020 3rd Option Period 1 October 2020 30 September 2021 4th Option Period 1 October 2021 30 September 2022 4. PLACE OF PERFORMANCE: William Jennings Bryan (WJB) Dorn, Veterans Affairs Medical Center, 6439 Garners Ferry Road, Columbia, South Carolina 29209. 5. DEFINTIONS: Miscellaneous Trash Combustible and noncombustible materials such as paper, wrappings, cloths, cartons, metal, glass, floor sweepings, plastic, grass cutting, tree trimmings, street sweepings, waste products of fuels, wastepaper, lumber, plastics, etc. Garbage Coffee grounds, tea leaves, banana peels, citrus rings, onion bulls, and similar general kitchen scraps. Construction material material normally used in construction projects, to include wood, metal, wiring, piping, insulation, carpeting, etc. Yard debris material normally associated with maintenance of exterior grounds, including grass cuttings, tree trimmings, limbs, wood, etc. Recyclable waste consists of plastic bottles, plastic wrap, aluminum cans, etc. 6. DAYS AND HOURS OF OPERATION: Service for the 8-yard and 20-yard containers will be between the hours of 10:00 a.m. and 2:00 p.m. on days designed for each line item. Service for the 34-yard compactors will be between the hours of 9:00 a.m. and 11:00 a.m. on days designated for this line item. The empty compactor will be switched for the full compactor so that there is no interruption in compactor availability to the facility. The contractor will be required to remove all miscellaneous trash, nonedible garbage and any other waste materials from designated pick up locations: a. Back Access Road, across from building 36 (20 cubic yard container) Miscellaneous Trash and Garbage Pick up: Every Thursday b. Building #100 (20 yard container) used for Miscellaneous trash, construction debris Pick up: Tuesday and Thursday c. Building #100 (34-yard compactor - VA Property) used for Miscellaneous trash, garbage Pick up: Tuesday and Thursday d. Back Access Road, across from building 36 (20 yard container) used for yard debris Pick up: Service as needed (on call within 24 hours) f. Building 103 / 103A (8 yard container) used for miscellaneous trash and garbage Pickup: Tuesday and Thursday g. Storage Unit at Veterans Road storage Complex (8 yd container) used for excess furniture Pickup: Weekly (day of week to be determined but to be same day each week) h. Building 7 20 yd container for excess furniture and other warehouse items Pickup: Every two weeks (day of week to be determined but to be same day each week) i. Building 100 (8 yard container, behind medical waste trailer) used for collection of recyclable waste Pick up: Weekly (day of week to be determined but to be same day each week) 7. SANITARY CONDITIONS: The contractor will be required to dispose of the miscellaneous trash and non-edible garbage in such a manner as not to cause conditions detrimental to public health or to constitute a public nuisance. The contractor will assume full responsibility of compliance with all Federal, State, City and County laws, rules and regulations governing removal of waste material. Collections must be made in contractor containers (with the exception of the 34-yard compactor) which must be fully enclosed, rat-proof, leak-proof, fire retarding, kept clean and sanitized, and deodorized after dumping. The contractor is responsible for leaving pick up areas around and under each container and the 34-yard container or compactor in a clean grease-free, safe and sanitary manner. The contractor will be responsible for removing any trash and/or garbage that may be scattered about the area at the time of pick up. 8. REPAIRS: The contractor is responsible for repairs to all containers issued and any damages to the 34-yard compactors. 9. SIZE AND TYPE OF CONTAINERS: The container shall furnish the necessary steel container of the size listed (with the exception of the two (2) - 34 yard compactors which are the property of the VA) in the area designated by the Chief or Assistant Chief, Environmental Management Service. 10. APPROXIMATE ACCUMULATION: Approximately 150 cubic yards of trash and garbage will be accumulated daily and will require removal. In FY2016, this equaled to approximately 800 to 900 tons of trash and garbage. The approximate accumulation total is furnished for information estimating purposes only. The contractor agrees to remove the entire accumulation of trash and garbage at the pickup price quoted herein. If any additional charges are applicable for excess weight or yardage, cost must be included in quote. Any estimated revenue from recycling containers can be used to offset cost in contract bid price. 11. POINT OF CONTACT: The contractor shall provide the name and telephone number of an individual to act as his representative and be responsible for coordination of the contract with the Government. 12. INVOICES: Payment will be made upon receipt of a properly prepared detailed invoice, prepared by the Contractor, validated by the Contracting Officer s Technical Representative (COTR), and submitted to VA FSC, P. O. BOX 149971, AUSTIN, TX 78714. A properly prepared invoice will contain: Invoice Number and Date Contractor s Name and Address Accurate Purchase Order Number Supply or Service provided Total amount due 13. RECORDS MANAGEMENT LANGUAGE FOR CONTRACTS The following standard items relate to records generated in executing the contract and should be included in a typical Electronic Information Systems (EIS) procurement contract: Citations to pertinent laws, codes and regulations such as 44 U.S.C chapters 21, 29, 31 and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act (5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228. Contractor shall treat all deliverables under the contract as the property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records. Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. Contractor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract. The Government Agency owns the rights to all data/records produced as part of this contract. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data. Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.] or state of completion [draft, final, etc.]. No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules. Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship (sub-contractor) in support of this contract requiring the disclosure of information, documentary material and/or records generated under, or relating to, this contract. The Contractor (and any sub-contractor) is required to abide by Government and Agency guidance for protecting sensitive and proprietary information.
