- Provisions & Clauses NIH Invoice Submission-Instructions Statement of Work Solicitation Number: HHSN27320170731 - Tapes Storage and Transport This is a Combined Synopsis/Solicitation for Commercial Items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice and in conjunction with the procedures at FAR Subpart 13.106. This Request for Quotation (RFQ) announcement constitutes the only solicitation that will be issued; quotes are being requested and a written solicitation will not be issued. The Government contemplates a Firm-Fixed Price Purchase Order will result from this RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. The applicable North American Industry Classification System (NAICS) code for this acquisition is 561990 "All Other Support Services", Size Standard - $11,000,000.00. The NIEHS has a requirement for: Tapes Storage and Transport Period of Performance: 09/19/2017 - 09/18/2018 plus four (4) options years. a.Option Year One = 9/19/2018 to 9/18/2019 b.Option Year Two = 9/19/2019 to 9/18/2020 c.Option Year Three = 9/19/2020 to 9/18/2021 d.Option Year Four = 9/19/2021 to 9/18/2022 Instructions to Offerors The National Institute of Environmental Health Sciences issues this Request for Quote (RFQ) for the purchase of: Qualstar Tape Library Maintenance. (Please see Statement of Work for the Line Items listing and for the Tape Serial Numbers.) This requirement is for the NIEHS, Office of Management (OM), Computer Technology Branch (CTB). Pickup Location: NIEHS Main Campus - 104 Building, Data Center, NIEHS, 111 T.W. Alexander Drive, RTP, NC 27709 Basis of Award: Award will be made on the basis of the lowest price technically acceptable (LPTA). LPTA will be determined by the total price of the items quoted. Set-Aside: 100% Small Business Set-Aside. As part of the quote, provide a copy of any End User License Agreement (EULA), Terms of Service (TOS), or other similar legal instrument or agreement. Note that FAR Clause 52.212-4, including paragraph (u), applies to any Government Purchase Order resulting from this RFQ. Purchase Order Type: Firm-fixed price (FFP). List of Attachments: 1.
