- SF-1449 W91QVN-17-R-0134 IMLPN OSAN AIR BASE This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested IAW with the attached SF1449. (ii) Solicitation number W91QVN-17-R-0134 is issued as a Request for Proposal (RFP). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95 effective 01-13-2017. (iv) The selected NAICS code is 621399 Offices of All Other Miscellaneous Health Practitioners. Small business size standard is not applicable. Set-Aside not applicable. (v) A list of line item number(s) and items, quantities, and units of measure (Reference attached SF 1449 Section B - Supplies or Services and Prices). (vi) Description of requirements for the items to be acquired (Reference attached SF 1449 Section B - Supplies or Services and Prices and Section C - Performance Work Statement). (vii) Date(s) and place(s) of delivery and acceptance and FOB point (Reference attached SF 1449 Section F - Deliveries or Performance). (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. Addendum to FAR 52.212-1 also applies to this acquisition (Reference attached SF 1449 Section L - Instructions, Conditions, and Notices to Bidders).
