- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Surgical Instrument Maintenance Program (SIMP) is to provide maintenance, repair, and training of surgical instruments utilized by clinics and operating rooms which are not limited to minimally invasive surgery instruments and micro instruments. This service will provide support to ensure the instruments are maintained in operational condition. The service supports the Davis Grant Medical Center (DGMC) mission by maintaining standards and the availability of surgical instruments and scopes so that key medical care and operation is not delayed for the DGMC staff at Travis AFB. Documents submitted in response to this solicitation must be fully responsive to be eligible for an award and consistent with the following Requirements of the solicitation (Contract Line Items (CLINs) and Performance Work Statement, and government standards and regulations pertaining to the PWS. AS STATED IN SOLICITATION ADDENDUM TO 52.212-1 PROPOSAL PREPARATION INSTRUCTION The response shall consist of three (3) separate parts: Part I - Price Proposal Part II - Technical Proposal; Subfactor 1, Labor: •(a) Include Phase-In Plan, if required •(b) Staffing Plan •(c) Indicate ability to provide a high powered diagnostic camera, up to 50X magnification Specta-Scan and magnification of up to 250X pictures of Spectra-Scan •(d) Staffing Matrix (with supported redacted Resumes to demonstrate requisites to perform PWS tasks •(e) Identify any potential Organization Conflicts of Interest •(f) Submit copy of certification for the following; American Society for Quality (ASQ), Certified Quality Inspectors (CQI), International Association of Healthcare Central Service Material Management (IAHCSMM) and International Standards Organization (ISO) 9001:2008:13485. •(g) Submit QCP Subfactor 2, Descriminators/Experience: •(a) Include list of three (3) relevant projects performed in the last three years and provide all information required for each project submitted in Addendum to 52.212-1, C. 2. (b) (i)-(x) Part III - Subcontracting Plan (Large Businesses Only) SITE VISIT NOTICE: An organized site visit has been scheduled for: Date: 14 August 2017 Time: 10:00 AM PST Location: 101 Bodin Circle Building 777, Room 2C240.1 Travis AFB, CA 94535-1800 Please contact tricia.keene@us.af.mil to coordinate your base access not later than 12:00PM PST 4 August 2017 to attend this Site Visit. All questions regarding this solicitation shall be received no later than 2:30PM PST 14 August 2017.
