Attachment 4: Past Performance Questionnaire Attachment 3: Subcontractor Consent Form Attachment 2: DD254 (DRAFT) Attachment 1: Performance Work Statement for FA6800-17-Q-S032. Combined Synopsis Solicitation for Fixed Based Weather Observing System (FBWOS) Sustainment. AFICA/KGQ at Barksdale Air Force Base intends to solicit competitive proposals to provide support for sustaining the OS-21 weather sensor suite of equipment at Air Force Global Strike Command (AFGSC) missile sites at a high level of operational capability in support of ground and flight safety operations supporting missile launch operations. AFGSC supports the National Command Authority and U.S. Strategic Command offensive and defensive inter-continental missile strike forces, a Force Activity Designator (FAD) 1 mission. Weather sensors are a required component of the crew flight safety operations supporting this weapon system, and provide real-time local weather information input to the launch complexes in order to support ground movements and rapid-response helicopter flights for crews responding to launch alerts. Failure to maintain the systems at full operational capability impacts launch capabilities and crew safety. This service is required on a continual basis, 365 days, 24/7, and this task order effort covers a five-year support period to minimize potential loss of support during contract changes, and avoids the high cost for another contractor to become trained on the equipment by the vendors, establish parts inventories, establish site access, and perform immediate work. The contract period of performance include one (1), 12-month base period with four (4), 12-month options periods.
