Description
Image Guided Surgical Table Request This request for information/sources sought notice is for planning purposes ONLY. The results of this market research will assist in the re-evaluation of the acquisition strategy in accordance with recent VA Procurement Policy guidance. VA assumes no responsibility for any costs incurred associated with the preparation of responses submitted as a result of this notice and does not guarantee an RFQ will follow. Veterans Health Affairs, Network Contracting Office 4, is issuing this request for information/sources sought notice in order to identify small businesses, and specifically, Service-Disabled Veteran-Owned Small Businesses (SDVOSB) or Veteran-Owned Small Business (VOSB) sources that have the resources and capabilities to provide one (1) Brand Name or Equal Steris 6000 SurgiGraph Image Guided Surgical Table and ancillary items for the Philadelphia, PA VA Medical Center. Salient Characteristics Designed for minimally invasive endovascular, vascular, and pain management procedures requiring intra-operative fluoroscopic C-Arm imaging Patient Weight Capacity 500 lbs. Powered Floor Locks. Self-compensating floor locks that establish 8 points of contact with the floor for extra stability Lateral Tilt to the right and left 16 degrees Table top length 92 Table top width 21 Carbon filter table extension Surgeon has complete control of table movement with hand control. Palm control is critical to movement of table. Table floats and can be controlled with one hand. (No JOY STICK) Non-motorized float feature must be included in Palm Control Table that enables radiology-quality imaging, lateral and longitudinal float, Trendelenburg and reverse Trendelenburg, lateral tilt, height adjustment Unobstructed Imaging Length of table Casters are enclosed to prevent accumulation of contaminants Base and covers are brushed surgical stainless steel Easy to clean surface Include Standard Accessories Catheter Extension Side Rail Extender Radial AV Fistula Board {including pad & pair of side rail locks STATEMENT OF WORK Scope of Work: Contractor shall provide all labor, personnel, equipment, tools, materials, supervision tools and equipment necessary to properly and safely perform the services set forth within this statement of work. Service Requested: Inspect and Start-Up Image Guided Table Stage the table in the location designated by the Government for review and check-in by the facility. Uncrate the table Ensure that power is fully charged for an all-systems check; Fully inspect table condition and supporting hardware; Perform and complete start up test to confirm optimal table performance; Verify floor locking system is secure; Confirm tilt speed; Inspect shroud to ensure complete functionality and verify no damage during freight transportation. Remove and properly dispose of all crating material. Provide two {2} copies of all associated operational and/ or service manuals Contractor Compliance: Provide all Personal Protective Equipment (PPE) which may be required for the work to be completed in a safe manner. Comply with VA security management program and obtain the permission of the VA police, be identified by project and employer, and restricted from unauthorized access. Comply with and be responsible for the safe removal and disposal of all packing materials and related waste generated during inspection/Start-up Task Frequency and Instructions: The work set forth in the Scope of Work shall be completed in a single visit, be it a single day or multiple days as required for appropriate completion. All work is to be scheduled in advance with Coordinate Inspection dates in advance with Logistics, Bio-Medical Engineering Special Work Requirements: The Contractor shall comply with any and all Infection Control Risk Assessment requirements deemed appropriate by the VA Medical Center during all portions of the work set forth in the Scope of Work. Property Damage: The contractor shall take all necessary precautions to prevent damage to any government property. The contractor shall report any damages immediately and shall be assess current replacement costs for property damaged by the contractor, unless corrective action is taken. Any damaged material (e.g. trees, shrubs, lawn/turf, curbs, gutters, sidewalks, etc.) will be replaced in a timely manner or corrected by the contractor with like materials, at no extra cost to the government upon approval of the Contracting Officer. Identification, Parking, Smoking, Cellular Phone use, and VA Regulations: The contractor's employees shall wear visible identification at all times while on the premises of the VA property. It is the responsibility of the contractor to park in the appropriate desig nated park ing areas. Information on parking is available from the VA Police-Security Service. The VA will not invalidate or make reimbursement for parking viola tions of the con tractor under any con ditions. Smoking is prohibited inside any buildings at the VA. Cellular phones and two way radios are not to be used within six feet of any medical equipment. Posses sion of weapons is prohibited. Enclosed containers, including tool kits, shall be sub ject to search. Violations of VA regulations may result in a citation answer able in the United States (Federal) Dis trict Court, not a local district state, or municipal court. Compliance with OSHA Blood borne Pathogens Standard: The Contractor shall comply with the Federal OSHA Blood borne Pathogens Standard. The contractor shall have methods by which all employees are educated as to risks associated with blood borne pathogens. Have policies and procedures which reduce the risk of employee exposure to blood borne pathogens Have mechanisms for employee counseling and treatment following exposure to blood borne pathogens Provide appropriate personal protective equipment/clothing such as gloves, gowns, masks, protective eyewear for the employee during performance of the contract.
