Description

STATEMENT OF WORK GENERAL INFORMATION 1. Title of Project: Purchase 5 (ea) AMSCO Steam Sterilizers from STERIS. 2. Project Scope: Provide equipment to be shipped to MEDVAMC, 2002 Holcombe Blvd. Bldg. 100, Warehouse Houston, TX 77030. The contractor shall provide all resources necessary to accomplish the deliverables described in the statement of work (SOW), except as may otherwise be specified. The VAMC facilities service line will make all necessary utility disconnections. The overall price should include the cost for delivery, trade-in for the five steamers that are being deinstalled, deinstallation/installation and disposal for the following items: Equipment removal and disposal shall include the following: 3. The equipment is in the following locations: Bldg. 110 - Room 138 (1st Floor) Bldg. 109 - Room 221C (2nd floor) Bldg. 109 -Room 111 (1st Floor) Bldg. 109 - Basement hallway near B10 Bldg. 109 - Basement #B37 4. Removal / Disposal of (4) Deinstallation - Medium Sterilizers (1) Deinstallation - 20 Inch Steam Sterilizer 5. Installation shall include the following: AMSCO ® 250LS Small Steam Sterilizer, 20" x 20" x 38": - 2 EA Loading Rack & Two Shelves-20X20X38" Sterilizer For Single Door Unit Only 20 Inch Lab 250(LS) Steam Sterilizer SF521301111000000F AMSCO ® Amsco ® Century ® Medium Scientific Laboratory Steam Sterilizer 26 X 37.5 X 60" - 2 EA 60" Atlas Loading Car And Transfer Cart For Hinged Platform Installation - 26 X 37.5 Inch Steam Sterilizer SF2310221120000A0F Amsco ® AMSCO ® 110LS Small Steam Sterilizer, 16" x 16" x 26" 1 EA Installation - 16 Inch Lab 110(LS) Steam Sterilizer SF511301111000000F AMSCO ® 6. Performance Period: NTE 180 Days from receipt of award. 7. Type of Contract: Firm-Fixed-Price 8. Place of Performance: MEDVAMC 2002 Holcombe Blvd. Bldg. 109, Rooms 111, 221C, B7, and B37 and Bldg. 110, Room 138 Houston, TX 77030 9. Site Visit Site Visit is scheduled for Wednesday, July 26, 2017 beginning promptly at 9 o clock at the VAMC Houston and all interested contractors should gain access to the VA through Entrance # 74 at 2002 Holcombe Blvd because the buildings where the equipment is located is not in the main hospital but you be escorted to the department where the work will be conducted. B. CONTRACT AWARD MEETING The first kick meeting will be scheduled through the COR 30 days after the award. (The COR will be identified upon completion of the award) C. GENERAL REQUIREMENTS D. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES Description of Tasks and Associated Deliverables: The contractor shall provide the specific deliverables described above in A. 2 within the performance period stated in Section A.4 of this SOW. Task One: Provide equipment, installation and one site training consisting of how to operate and program the cycles. Deliverable: Five (5 each) AMSCO Steam Sterilizers (Please see above in A.5 for the description of individual steam sterilizer). E. SCHEDULE FOR DELIVERABLES All deliveries or work shall be conducted Monday Friday 8am 5 pm unless otherwise noted. No deliveries or work shall be performed/accepted on weekends and holidays. If for any reason the scheduled time for a deliverable cannot be met, the contractor is required to explain why (include the original deliverable due date) in writing to the CO, including a firm commitment of when the work shall be completed. This notice to the CO shall cite the reasons for the delay, and the impact on the overall project. The CO will then review the facts and issue a response in accordance with applicable regulations. F. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. J. GOVERNMENT RESPONSIBILITIES The government will provide controlled access to the areas and other resources if required to perform the services. The government will assign a primary POC, and to provide information and resources in a manner to maintain contract continuity. Additionally, this person will receive deliverables as a result of this contract. M. ARCHITECTURAL GUIDELINES None N. SECURITY- 1. After award, contractors will summit flash badges applications to be certified vendors to operate on the VAMC campus to the appointed COR and driver s licenses are needed to complete the form. The flash badges applications will be supplied by the COR. Evaluation Criteria FACTOR 1 - TECHNICAL The Offerors must submit an official letter from Steris that shows they are an authorized distributor of Steris' equipment and proof that their technicians/installers are trained by Steris. FACTOR 2 PRICE The quotes will be evaluated using the lowest price technically acceptable evaluation process. The government reserves the right to only evaluate the lowest priced bid for acceptance. If that bid is found to not be technically acceptable, the government will move forward with the next lowest offeror and continue in that manner, as applicable.
