Description

This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in Subpart 12.6, and is being conducted under FAR Subpart 13; simplified acquisition procedures apply. VA will conduct the procurement according to a combination of FAR Part 12, Acquisition of Commercial Items and Part 13, Simplified Acquisition Procedures as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. The solicitation number is VA250-17-Q-0879 and is issued as a Request for Quotation (RFQ) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-69, August 1, 2013. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at http://www.arnet.gov/far (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR) This solicitation is issued as a small business set aside. The North American Industry Classification System (NAICS) code is 339113, and the size standard is 750 employees. Any contractor that believes they are capable and desires to claim preference for small business status must be registered with the SBA at http://web.sba.gov/pro-net/ and meet the requirements of FAR 19.102. Any contractor that believes they are capable and desires to claim preference for veteran owned small business status must be registered with the VIP at https://www.vip.vetbiz.gov as an SDVOSB or VOSB. A Contractor that proposes to provide a supply that is not manufactured by their organization (nonmanufacturer) shall meet the requirements of FAR 19.102(f). Contractors that deem themselves capable of meeting the requirement shall provide the below information to Michael Iffland, Contract Specialist, at michael.iffland@va.gov. RESPONSES ARE DUE, no-later-than Thursday July 27, 2017 at 5:00PM, EDT. Electronic offers will be accepted. Responses shall include: (1) Business Name and Address, (2) GSA Contract Number, if applicable (3) Point of Contact Name, Phone Number and E-mail Address (4) DUNS and NAICS code (5) Business Size SMALL or LARGE (6) Type of Business: service disabled veteran owned, veteran owned small business, 8a, HUBZone, woman-owned, etc. (7) A concern that is not a manufacturer of the supply shall provide the manufacturers name and size standard. If the concern is the manufacture and the sole distributor, please provide documentation to that effect. Delivery is to: VA Northern Indiana Health Care System - Marion Campus 1700 East 38th Street Marion, IN 46953 Installation location: VA Northern Indiana Health Care System-Fort Wayne Campus 2121 Lake Ave. Fort Wayne, IN 46805 The contractor shall supply and install: ITEM NUMBER DESCRIPTION OF SUPPLIES/MODEL QUANTITY UNIT 0001 Reverse Osmosis System / AW00HC2062 1.00 EA 0002 Water Blend Valve / AW00HCS5001 1.00 EA 0003 Installation 1.00 EA We are looking for Brand Name or equal that has the Salient Characteristics listed below: Salient Characteristics The VA Northern Indiana Health, Ft Wayne, SPS care system has a need for a Reverse Osmosis water system with blend valve to do final rinse by hand or washer, with the following specifications: High flow pump 100gal tank Blend valve Pre-carbon block filter Anti-scalant feed to prevent hard water scale on RO Controller with conductivity of feed and product water Simple disinfection cycle Low feed pressure shut down Storage tank with sealed lid and pump package Deionizer polisher with a 200,000 ohm-cm monitor with alarm. Warranty to begin upon completion of installation Water blend valve: To be used in conjunction with RO system to maintain the incoming water temp at 77F. 52.212-1 Instructions to Offerors - commercial Items, apply to this solicitation with the following addenda, 52.203-98, 52.204-16, 52.204-17 52.212-2, Evaluation - Commercial Items: FAR provision 52.212-2 does not apply to this solicitation.
