- Page 1 of 1 Special Notice Request for Information Only. The Great Lakes Acquisition Center (GLAC) NCO 12, on behalf of the Hines VA Hospital, 5000 S. 5th Ave. Hines, IL 60141-1457, herein after called VA is looking for certified Service Disabled Veteran Owned Small Business (SDVOSB) and Veteran Owned Small Business (VOSB), Small Business or Large business sources for the following: Long Term Acute Care (LTAC) Services: The contractor shall provide LTAC services, at the contractors site for patients with complex needs and possible comorbid conditions including mechanical ventilation bed support, Intravenous (IV) Infusion, Antibiotic Therapy, Total Parenteral Nutrition (TPN) support, and complex wound care in an acute hospital setting as necessary. Services shall be provided by an interdisciplinary team of clinicians necessary for involvement in the patient s care and recovery. The contracted facility should provide effective Long Term Acute Care Services for patients recently referred from a VA facility, directly to the LTAC hospital. The Dept. of Veterans Affairs anticipates the awarding of three LTAC contracts within the following catchment area: Madison VA Hospital, 2500 Overlook Terrace, Madison, WI 53705-2254 Jesse Brown VAMC, 820 S. Damen Ave, Chicago, IL 60612-3728 Hines VA Hospital, 5000 S. 5th Ave. Hines, IL 60141-1457 Responses to this announcement must be submitted in writing to the Contracting Officer no later than 10:00AM Central Time, July 28, 2017 to the following email address: Patrice.Bond@va.gov and Prudence.Holland@va.gov, No other form of communication will be accepted. Sources interested in responding to this notice are required to submit a capability statement that includes experience, license, certifications, and provide the distance from its location to the VA facility. A determination by the Government not to compete based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purposes of determining whether to conduct a competitive procurement or to proceed with sole source contracts. This is for market research purposes only and is not a request for competitive proposals; however, any firm that believes it can meet these requirements, please provide written notification to the Contracting Officer. SDVOSB or VOSB must be certified and verified in the U.S. Department of Veteran Affairs, Office of Small & Disadvantaged Business Utilization, Vendor Information Pages web site located at https://www.vip.vetbiz.gov/. Any response to this notice must show clear and convincing evidence that competition would be advantageous to the Government.
