Description
The Department of Veterans Affairs, Network Contracting Office 10, 24 Frank Lloyd Wright Drive, Lobby M, Suite 2200, Ann Arbor, MI 48105, intends to solicit offers for Disposal of Infectious Waste Services for the Ann Arbor VA Medical Center and Community Based Outpatient Clinics. Contract will be for the period October 1, 2017 through September 30, 2018 with four (4) option years in strict accordance with terms, conditions and provisions of the contract solicitation, NAICS Code 562112. Contractors that can provide this requirement and meets the requirements below are requested to send their 1. Company Name, 2. DUNS number, 3. Business size under NAICS 541614, and 4. If the item(s) are available on a FSS contract or not and the FSS contract number if it is available on contract, 5. Notification if they are a VETBIZ registered VOSB/SDVOSB or not. Contractors must be registered and certified in VETBIZ to be considered VOSB/SDVOSB. This is not a solicitation. This is a Request for Information, Sources Sought. This Sources Sought will close on 07/21/2017 at 12PM EDST. Please e-mail all responses to christopher.gundy@va.gov. Statement of Work Scope General: The Contractor shall provide all labor/personnel, waste processing, equipment, supplies, packaging, storage (i.e., 40-foot trailer), manifests, and transportation services necessary for the disposal of the waste in accordance with all local, state, and federal regulations, specifically Chapter 333 MI public Health Code Part 138 and Ohio Administrative Code (OAC) 3745-27; and all terms and conditions of this contract. All red-bagged items, regardless of their condition, are being discarded by the Government and are considered to be infectious waste. Pickup at the Ann Arbor (two locations), Jackson, Flint and Toledo CBOC facilities shall be bi-weekly, with the exception of the MHICM/NAC facility which is monthly, or as determined by the Contracting Officer Representative (COR) with removal commencing no later than 2:00 p.m., Monday through Friday (excluding Federal Holidays) at the Receiving Dock. Evaluation factors are technical and past performance. Department of Transportation Requirements: Transportation shall be in accordance with DOT HM-181. The Contractor shall also comply with state and local requirements, including obtaining all necessary permits, licenses, and approvals. Spill Reporting Requirements: The Contractor is responsible for reporting to the Contracting Officer all spills, regardless of quantity, and all personnel exposures. Such reports shall first be reported by telephone immediately following the incident and shall be followed up in writing no later than seven (7) days after the initial report. When reporting a spill, the following information shall be furnished: (1) quantity spilled; (2) exact location of spill; (3) containment procedures initiated; (4) clean-up and disposal procedures. Permits and Responsibilities: The Contractor shall, without additional expense to the Government, be responsible for obtaining any necessary licenses and permits, and for complying with any applicable Federal, State, and Municipal laws, codes, and regulations in connection with the prosecution of the work. This includes acquiring any installation listed in this contract. Contractor shall be similarly responsible for all damages to persons or property. Contractor shall take proper safety and health precautions to protect the work, the workers, the public and the property of others. The Contractor shall also be responsible for all materials delivered and work performed until completion and acceptance of the entire work.
