Description
Wage Determination PWS Personnel Relocation Combined Synopsis/Solicitation This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Federal Acquisition (FAR) Subpart 12.6 (reference http://farsite.hill.af. mil ) as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation # Solicitation Purpose FA2835A170005 This solicitation is a request for quotation (RFQ). This will be a Blanket Purchase Agreement (BPA) Requirement: Personnel Relocation Projects Hanscom Air Force Base (AFB) is located in Bedford Massachusetts, Hanscom AFB is approximately 20 miles Northwest of Boston Massachusetts and 20 miles Southeast of Harvard Massachusetts. Nearby cities surrounding the base include Lincoln, Sudbury, Lexington, Lowell, North Chelmsford, Woburn, Waltham, Watertown, and Newton Massachusetts. Hanscom Air Force Base (66 th ABG) provides worldwide support for the Air Force Life Cycle Management Center (AFLCMC), one of five centers under the Air Force Materiel Command (AFMC). The AFLCMC is the single center responsible for total life cycle management of Air Force weapon systems. The AFLCMC mission is to deliver affordable and sustainable war-wining capabilities to U.S. and international partners on time, on cost, anywhere, anytime, from cradle to grave. Hanscom AFB Installation personnel - as well as some Hanscom AFB personnel located on the MITRE campus (located at 202 Burlington Road Bedford Massachusetts 01730) - may be required to relocate either within their current buildings or move to a different buildings within the Hanscom Air Force Base (AFB) Installation located in Bedford Massachusetts 01731. Vendors shall submit their offers in writing, and do so using company letterhead or other formal means of documenting the offer which clearly indicates company information and clearly documents offer; •2. The vendor shall submit signed and dated offers to the assigned PZIB Contract Specialist, Sharley Robertson at sharley.robertson@us.af.mil, on or before the exact time and date specified in this solicitation; •3. The item(s) to be quoted are listed in TABLE-1, p.11 of the performance work statement (PWS). Please place your quoted cost in the "UNIT Cost" columns provided in TABLE-1, and place the total cost in the "Total Cost" column of TABLE-1; •4. Warranty: A clear description of the terms of any applicable warranty; •5.9. Price/Discount: Clearly document the requested pricing information, as well any applicable discounts; •5.10. Remit To Address : Clearly indicate/specify the vendor's correct "Remit To" address, •5.11. Past Performance Information: Include (as appropriate0 recent and relevant contracts for the same or similar items, as well as references (as appropriate) including contract names and numbers, and •5.12. General: The offeror shall include a statement specifying the extent of agreement with all terms, conditions, provisions, and clauses included in this solicitation. •6. FAILURE TO COMPLY Offers that fail to furnish information required by this document; are inactive in the Government's SAM database; or reject the terms and conditions set forth in this combined synopsis/solicitation may be excluded from consideration. 52.212-2, Evaluation -Commercial Items In accordance with Federal Acquisition Regulation (FAR) 15.101-2, respondent's offerors will be selected based on the lowest price technically acceptable process. The "Requirements" for this evaluation are set forth in the Performance Work Statement (PWS) entitled: Hanscom Air Force Base Personnel Relocation Project. 52.212-3, Offeror Representations and Certifications - Commercial Items Offerors are advised to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with their proposal(s). Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation. (b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver's license, current vehicle registration, valid vehicle insurance certificate, and to obtain a vehicle pass. (c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site. (d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with AFI 31-101, Integrated Defense, and AFI 31-501, Personnel Security Program Management ] citing the appropriate paragraphs as applicable. (e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office. (f) Failure to comply with these requirements may result in withholding of final payment. (End of clause) AFFARS 5352.201-9101: Ombudsman; (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions). (c) If resolution cannot be made by the contracting officer, the interested party may contact the ombudsman, [Insert names, addresses, telephone numbers, facsimile numbers, and e-mail addresses of Center/MAJCOM/DRU/AFRCO/SMC ombudsman/ombudsmen]. (b) The contracting officer may, by written order, direct Air Force Occupational Safety and Health (AFOSH) Standards and/or health/safety standards as may be required in the performance of this contract and any adjustments resulting from such direction will be in accordance with the Changes clause of this contract. (c) Any violation of these health and safety rules and requirements, unless promptly corrected as directed by the contracting officer, shall be grounds for termination of this contract in accordance with the Default clause of this contract.
