Description
Limited Sources Justification FOR THE PURCHASE OF Platinum Elite DX and accessories ITSS RFQ NUMBER ID05170084 A procurement by the U.S. General Services Administration on behalf of The United States Air Force, 711th Human Performance Wing This requirement is being fulfilled through a one-time delivery order NAICS: 339112 - Surgical and Medical instrument manufacturing   1.Agency and Contracting Activity Requiring Agency:711th Human Performance Wing 2510 5th Street WPAFB, OH 45433 Contracting Activity:General Services Administration, Region 5 Federal Acquisition Service Contracting Division 2600 Paramount Place Ste 180 Fairborn, OH 45324 2.Nature and/or Description of the Action Being Approved System will be used in conjunction with current box already utilized to help with increase in patient care. Recently tripled the # of patients requiring full pulmonary function studies. The increase is attributed to a study being performed to identify correlation between aviators born premature (Prior to 36 week gestation), and decrease in lung development/function. Current capabilities allow us to see 5 a day, but with another box that would allow us to see 10-12 a day. Having the CPFSD allows for testing to be performed anywhere, and is capable of performing all tests (except for those requiring to be sealed in the box TGV and Raw). This is particularly useful in for MCT cases. Our current capabilities only allow us to see 2 MCT patients a day, and with increased patient load many patients are being delayed until late in the week or have even had to be sent to the 88th to have the test performed. The reason for this is that we have to allow for the room to properly ventilate due to the toxicology of the drug used for several hours before the next exam can be performed. The CPFSD allows for MCT testing to be performed in any available exam area without the added delays in care due to it being a self-sustained system on a mobile cart. This increases our capabilities from 2 a day to 6 or more if rooms are available to utilize. 3.Description of the Supplies/Services Required to Meet the Agency's Need This requirement is for a Platinum Elite DX Body Plethsmograph for the Internal Medicine Branch of the Aerospace Consultation Service. The Platinum Elite is a fully enclosed system capable of performing all required pulmonary function tests ;FVC, SVC, MVV, TGV, Raw, MIP/MEP, DLco, N2 Washout, SBN2, and can perform Bronchial Provocation studies) in accordance with ATS/ERS standards. 4. The Authority and Supporting Rationale and, if applicable, a Demonstration of the Proposed Contractor's Unique Qualifications to Provide the Required Supply Service Authority FAR 8.405-6(a)(1)(i)(B), only one source is capable of providing the supplies or services required at the level of quality required because the supplies or services are unique or highly specialized. The requiring agency needs to purchase this item one-time. The manufacturer has a GSA Schedule and purchasing from the schedule open market is more cost effective and advantageous to the Government. The manufacture has no other distributors of the product. 5. A Determination by the Ordering Activity Contracting Officer that the Order Represents the Best Value The manufacture's public price list will be compared to the GSA schedule pricing list. The Contracting Officer will determine that the price is fair and reasonable at the time of contract award. 6.Description of the Market Research Conducted or a Statement of the Reason Market Research was not Conducted The GSA Program Manager performed market research and found the manufacturer; MGC, Diagnostic has Schedule 84, Contract# V797P-4220B. The North American Classification System (NAICS) is 33912, Surgical and Medical instrument manufacturing. The manufacture is a small business under the NAICS. 7.Other Facts Supporting the Justification The Government believes that award to any other source would result in an unacceptable delay as well as substantial duplication of costs. 8. A Statement of the Actions, if any, the Agency may take to Remove or overcome any Barriers that Led to the Restricted Consideration before any Subsequent Acquisition for the Supplies or Services is made None 9. CONTRACTING OFFICER CERTIFCATION. I certify that the data supporting the recommended use of Limited Sources Justification is accurate and complete to the best of my knowledge and that the price is fair and reasonable.
