Description

COMBINED SYNOPSIS/SOLICITATION Medical Commodity Industry Pricing Data System Access This is a Combined Synopsis/Solicitation for Commercial Items prepared in accordance with the format in FAR Subpart 12.603 Streamlined Procedures for Evaluation and Solicitation for commercial items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. All responsible sources may submit a quotation which shall be considered by the agency. The acquisition is being made using simplified acquisition procedures. Solicitation number VA119-17-Q-0417 is issued for this combined synopsis/solicitation; this solicitation is being issued as a Request for Quote (RFQ). This combined synopsis/solicitation incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-95 dated January 19, 2017. This acquisition is solicited on an unrestricted basis utilizing North American Industrial Classification System (NAICS) code 519190, with a size standard of $27.5 Million. Attachment B Price Schedule contains a list of contract line item numbers and items, quantities, and units of measure. A description of the requirements is contained in Attachment A Performance Work Statement (PWS). Date(s) and place(s) of delivery and acceptance are also included in the PWS. 52.212-1 Instructions to Offerors-Commercial is applicable to this acquisition and the addenda in Attachment C - Instructions to Offerors applies; 52.212-2 Evaluation-Commercial Items is not applicable to this acquisition but specific evaluation procedures are detailed in Attachment D Evaluation of Quotes. Quoters shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with their quote (see Attachment E- Contract provisions and clauses). The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition and all additional FAR clauses cited in the clause are included in Attachment E- Contract provisions and clauses. There are no additional contract requirement(s) or terms and conditions necessary for this acquisition. The Defense Priorities and Allocations System (DPAS) are not applicable. All quotes are due on or before August 4, 2017 at 4:00 PM EST. Quotes shall be submitted via e-mail to Hosea.Berkley@va.gov and Blake.mcilvane@va.gov. Any question in reference to this RFQ shall be submitted to Hosea.Berkley@va.gov no later than 4:00 pm on 08/02/2017.
