- Amendment 03 7/28/2017 The purpose of this Amendment to RFQ VA242-17-Q-0762 is to communicate: FAR Clause 52.222-62, Paid Sick Leave Under Executive Order 13706 is not applicable to this requirement. It is hereby removed from the solicitation, and will not appear in the final contract. ____________________________________________________________________________________ 7/28/2017 The purpose of this Amendment to RFQ VA242-17-Q-0762 is to communicate: FAR Clause 52.222-41 Service Contract Labor Standards is not applicable to this requirement. It is hereby removed from the solicitation, and will not appear in the final contract. ____________________________________________________________________________________ 7/25/2017 Assumption - Based on the description provided in the solicitation, it is assumed that this request is for Cath only. Please confirm this assumption is accurate. YES Based on this assumption, the following questions are submitted: 1. How many DICOM compliant modalites will be sending to CPACS?__2__ 2. What number of studies does the site perform per year? __~350_____ 3. Is a Zero-footprint Web viewer required? _____No______ 4. Does the site require/want LDAP authentication? ____No______ 5. What volume of studies will we be migrating? _3500-4000___ 6. Will the awarded vendor be the archived location for studies? _____No_____ 7. If Cath studies are currently dictated, will we receive an HL7 ORU with that report? ____No____ ___________________________________________________________________________________ 7/25/2017 1. Does the Government want vendors to provide a point-by-point response to the SOW requirements? Example: Page 5, Section D. OTHER, Must be able to HL7 interface Does Syracuse expect a response by line items that vendor complies or by virtue of our response is it assumed that the vendor meets compliance? Responses to this RFQ are assumed to meet this compliance by virtue of the response. However, if the specifications provided with the response indicate this may not be the case, clarification may be sought directly with the vendor. The awarded vendor is expected to meet all specification outlined in the SOW. If the SOW is not able to be upheld after award, contractual actions to terminate the contract may be sought. 2. Page 3, section (xi) relating to Quote packages must include, line items 5 and 6 a. Item 5 - Requests at least 2 VHA-related past performance references : Is there specific information you are look for relating to PP? Contract number and product utilized at that facility. b. Item 6 If proposing an equal system, : It assumes you want Customer site name, address, contact name, contact information and installed product. Is there additional information you are looking for related to references? Contract number or ID if available. 3. I didn t see reference to a 1449, will you be providing a 1449 or is Syracuse just looking for a quoted proposal for a solution that meets the SOW parameters? IAW with FAR Part 13, the combined Synopsis Solicitation as presented with the Statement of Work, is the complete solicitation/request for quote. Quotes are expected to conform to the parameters as outlined in the Statement of Work.
