ATTACHMENT 0006- INSTRUCTIONS TO OFFERORS ATTACHMENT 0005- ADDENDA TO SF 1449 ATTACHMENT 0004- STANDARD FORM (SF) 1449 ATTACHMENT 0003- PRICING MATRIX ATTACHMENT 0002- RESUME TEMPLATE ATTACHMENT 0001- STATEMENT OF WORK (SOW) This is a combined synopsis/solicitation for a commercial service prepared in accordance with the format in the Federal Acquisiton Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number DTFT6017R00014 shall be used to reference any written proposal provided under this Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular Current to FAC 2005-95. The United States (U.S.) Department of Transportation (DOT), Federal Transit Administration (FTA) is seeking contractor support regarding predicted versus analysis studies which will assist the Office of Planning and Environment (TPE). Contractor is to provide non-personal but professional services to the Government. The Government anticipates awarding one Labor Hour contract for this requirement. The total Period of Performance shall not exceed one year and six months consisting of one 12-month base period and one six-month option period (if exercised). In addition, the resulting Labor Hour contract shall include FAR Clause 52.217-8 Option to Extend Services (NOV 1999) as well. This procurement is a Total Small Business Set-Aside in accordance with the FAR 19.502. The North American Industry Classification System (NAICS) code for this project is 541611, PROFESSIONAL, SCIENTIFIC, AN TECHNICAL SERVICES - ADMINSTRATIVE MANAGEMENT & GENERAL MANAGEMENTCONSULTING SERVICES with a small business size standard of $15 Million. Attachments: 0001- Statement of Work (SOW) 0002- Resume Template (Must be used and submitted for each proposed personnel resume) 0003- Pricing Matrix (Must be filled out and returned as a Microsoft (MS) Office Excel spreadsheet) 0004- Standard Form (SF)-1449 Solicitation/Contract/Order for Commercial Items (to be filled out and returned with Price Schedule. Offeror must complete blocks 12, 17, 24, 24 and 30 on respective SF 1449 form) 0005- Addenda to SF 1449 0006- Instructions to Offerors FAR Provision 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition and the following addendum applies: Offerors must hold firm pricing for at least 60 calendar days. Offerors must include a completed copy of their Representations and Certifications with the offer in accordance with FAR Provision 52.212-3, Offeror Representations and Certifications- Commercial Items. Submission must be provided as part of Volume I. Submission of Representations and Certifications do not count towards the maximum page count for the respective Volume I. FAR clause at 52.212-4, Contract Terms and Conditions -- Commercial Items (JAN 2017), applies to this acquisition with the following Addendum: Paragraph (c) of this clause is tailored as follows: Changes in the terms and conditions of this contract may be made only by written agreement of the parties with the exception of administrative changes, such as changes in the paying office, appropriations data, etc., which may be changed unilaterally by the Government. The Government will award a Labor Hour contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Technical and Management Approach Past Performance Price Technical and past performance, when combined, are more important than price. There is a trade-off between non-price and price factors that will be considered when making the determination of best value. The Government intends to award a Labor Hour contract without discussions with respective offerors. However, the Government reserves the right to conduct discussions if deemed in its best interest. P lease ensure that any submission made in response to this Solicitation contains adequate information to allow for Government evaluation in accordance with the criteria. RFP response due date: Wednesday, August 16, 2017 RFP response due time: 1:00 PM, Eastern Standard Time (EST) Questions regarding this on RFP numbered DTFT6017R00014 must be received on or before 1:00 PM EST on Thursday, August 3, 2017 in order for the Government to provide timely responses. The Government may not respond to questions received after that the due date and time noted for questions in this announcement.
