Brand Name or Equal for the Pentacam AXL for the Minneapolis VA Health Care System NAICS Code: 339115 Notice Type: Sources Sought This is a Request for Information (RFI) only; it is not a request for quotations or offers. 52.215-3 -- Request for Information or Solicitation for Planning Purposes (Oct 1997) (a) The Government does not intend to award a contract on the basis of this solicitation or to otherwise pay for the information solicited except as an allowable cost under other contracts as provided in subsection 31.205-18, Bid and proposal costs, of the Federal Acquisition Regulation. (b) Although proposal and offeror are used in this Request for Information, your response will be treated as information only. It shall not be used as a proposal. (c) This solicitation is issued for the purpose of: conducting market research to identify potential sources for a potential procurement of Brand Name or Equal for the Pentacam AXL for the Minneapolis VA Health Care System. See Attached DRAFT Statement of Work This is a request for information and sources only, which may or may not lead to a future solicitation. This is not a request for proposal (RFP). The VA will not pay for any information received resulting from this sources sought notice. Information should be forwarded to the Contract Specialist. Any questions should be addressed to the Contract Specialist, in writing, at the email address provided. Because this is a request for information only, answers to questions will not be posted. If your organization has the capability to provide these items and is interested in this opportunity, please respond to Jody M. Product literature on items that are offered to meet the requirements of the DRAFT specifications (see attachment). Socioeconomic status. The deadline for this information is 12:00 PM Central Time on Friday, August 4, 2017. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. GREY MARKET ITEMS:   The Vendor shall be an Original Equipment Manufacturer (OEM) authorized dealer, authorized distributor, or authorized reseller for the proposed equipment/system such that the OEM warranty and service are provided and maintained by the OEM. The vendor shall provide new items ONLY; no remanufactured or "grey market" items. All items must be covered by the manufacturer's warranty.   The offeror shall provide evidence of its authority to distribute the proposed equipment/system at the time of submission of its quote/offer.   Please note that such evidence is subject to verification by the Contracting Officer at his/her discretion. DRAFT Pentacam AXL Procurement Project Title Pentacam AXL Procurement Background The Minneapolis VAHCS provides many services to its patient population. In order to provide effective and efficient patient care, a variety of equipment and systems are utilized in all of the departments. The eye clinic is one such area. Scope The Minneapolis VAHCS is procuring a Pentacam AXL retinal imaging system to be utilized by the eye clinic. Equipment shall have the following part numbers and brand name: Item # Part # Description Quantity 1 70100 Pentacam AXL 1 2 70718 DICOM for Pentacam 1 Equipment Requirements Contact-free from the corneal surface of the retina (contact with the eye is not needed to complete the study) Axial length measurements must be provided Scheimpflug Camera with integrated Optical Biometer Must be able to provide 3D images of the anterior segment in less than 2 seconds Head rest Chin rest Moveable base in x, y, & z directions Floating license Workstation to run equipment and associated software Must be Windows 7 or higher, if running a Windows OS Keyboard Wireless Mouse Table for equipment Software to include the following, at a minimum Pachymetry Densitometry 3D Chamber Analyzer Corneal Topography Belin-Ambrosio Enhanced Ectasia Holladay Report DRAFT Contact Lens Fitting 3D Cataract Analysis w/ Nucleus Staging Corneal Optical Densitometry Indices Report Axial specific IOL power calculations Automatic transfer of relevant data to the IOL. Blood vessels shown in green illumination Must be capable of the following formulas SRK/T Holladay 1 Hoffer Q Haigis PotvinShammasHill, after LASIK PotvinHill after RK Double-K formulas Meridional analysis General Requirements Weight must be no greater than 8kg ± 2kg Must be no larger than 11 ± 2 x 9 ± 2 x 17 ± 2 Digital CCD camera Blue LED light source 14-40 mm axial length 100 images in 2 seconds 3-38 mm curvature ±0,1 D precision and reproducibility 80 mm operating distance Training Contractor shall provide on-site go-live training during implementation. Contractor shall provide on-site technical training during implementation. Installation Contractor shall coordinate installation during normal business hours, with the approval of the COR. Contractor shall dispose of all waste. Contractor shall confirm full functionality of the with Minneapolis VAHCS staff before stating work is complete. Warranty Full two year warranty to include all parts and labor shall be included. Delivery Contractor shall provide the upgrade to the Minneapolis VAHCS, One Veterans Drive, Minneapolis, MN 55417.
