- Pricing Sheet QASP Non-Disclosure OCI Solicitation COMBINED SOLICITATION NOTICE This is a Combined Synopsis/Solicitation for Commercial Items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number W81XWH-17-T-0323 is issued for this combined synopsis/solicitation; this solicitation is being issued as a Request for Quote (RFQ). This solicitation is being conducted under FAR Part 13, Simplified Acquisition Procedures (SAP) and in conjunction with FAR Part 12, Acquisition of Commercial Items. The associated North American Industrial Classification System (NAICS) code for this procurement is 541620 - Environmental Consulting Services. The Small Business size standard is $15 million. Product and Service code (PSC) H999 - Miscellaneous Testing & Inspection Services. This procurement is a 100% set-aside for Service Disabled Veteran Owned Small Businesses (SDVOSB). The purpose of this contract is to provide U.S. Army Public Health Center (APHC) The contractor shall provide all personnel, equipment, tools, materials, and supervision to assist and support the U. S. Army Public Health Center (APHC) and Army installations worldwide with conducting and executing Industrial Hygiene(IH) baseline surveys (baseline workplace assessments), ventilation evaluations and/or noise surveys. The contractor shall be responsible for performing onsite IH workplace assessments at Army installations. The Army installations for which the contractor will provide services as referenced in the Statement of Work (SOW). The contractor will collect, evaluate and record this data into the Defense Occupational and Environmental Health Readiness System-Industrial Hygiene (DOEHRS-IH). A primary function of the Army Industrial Hygiene Program (IHP) is the data recording into the DOEHRS-IH website. The DOEHRS-IH is a web-based, comprehensive management information system for assembling, comparing, evaluating, storing, and extracting occupational personnel exposure information. Data such as workplace environmental monitoring, engineering and administrative controls, PPE, observation of work practices, and employee health hazard education and training are collected in DOEHRS-IH. DOEHRS-IH is the foundation for the federally mandated DoD longitudinal exposure record. The U.S. Army Medical Research Acquisition Activity (USAMRAA) intends to award a firm-fixed price purchase order. Award will be based on the lowest price technically acceptable quote. The Government reserves the right not to award a purchase order.
