Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6 "Streamlined Procedures for Evaluation and Solicitation for Commercial Items", as supplemented with additional information included on this notice. This announcement constitutes the only solicitation; offers are being requested, and a written solicitation document will not be issued. The Government intends to issue a purchase order in accordance with FAR Part 13 for using Simplified Acquisition Procedures. The solicitation number is VA247-17-Q-0773. This solicitation is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular FAC 2005-70 effective September 30, 2013. The associated North American Industrial Classification System (NAICS) code for this procurement is 337215 and Product Service Code (PSC) 7110. The evaluation of offers received in response to the solicitation will use a tiered or cascading order of precedence. Tiered evaluation of offers, also known as cascading evaluation of offers, is a procedure used in negotiated acquisitions when market research is inconclusive for justifying limiting competition to small business concerns or sub-categories of small business concerns. The contracting officer (1) Solicits offers from both small and other than small business concerns that will be evaluated in the following tier order: (a) service-disabled veteran-owned small business (SDVOSB); (b) veteran-owned small business (VOSB); (c) All other small business concerns; (d) Other than small business. (2) If an award or a sufficient number of awards cannot be made at the first tier, evaluation of offers will proceed at the next lower tier until an award or a sufficient number of awards can be made. The acquisition will be made pursuant to the authority in FAR 13 to use simplified procedures for commercial items. V-Vinyl Grade E-Grade Mome_VBR_Driftwood-Momentum, Bravo LL, Driftwood 1 Each INSTALLATION SET UP AND DELIVERY Statement of work Background: The Birmingham VA Medical Center (BVAMC) Main Campus located at 700 South 19th Street, Birmingham, Alabama wishes to replace the existing guest chairs with seventy (70) medical grade guest chairs to be used by patients, their families and staff throughout the Anniston CBOC located at 96 Ali Way Creekside South, Oxford, AL 36203 Phone: 256-832-4141. II. Purpose of Project: The existing seating has passed its useful life and has soiled fabric after years of use. They have now become a safety and infection control hazard. III. Statement of Work: This Statement of Work (SOW) defines the effort required by the Birmingham Veteran Affairs Medical Center (BVAMC) to establish compatible, uniform and safe patient seating by standardizing furniture requirements around existing pieces already in use throughout the facility. This effort will ensure that new furniture is compatible with existing furniture. Standardization of furniture will provide maximum flexibility to Engineering services who maintains the working order of furniture, Environmental Services who is charged with cleaning the furniture as well as create a streamlined and cohesive appearance so that items can be moved around the facility as needs change. A. Objective: The requirement under this SOW is to establish a single award contract to provide Health Centric Medical Grade Guest seating to be used by patients, their families and staff throughout the Anniston CBOC located at 96 Ali Way Creekside South, Oxford, AL 36203 Phone: 256-832-4141. B. Task / Scheduled Deliverables: 1. Contractor shall provide the furniture items required as described in the below salient characteristics. 2. Contractor shall act as the liaison between the BVAMC and the manufacturer if warranty issues arise. 3. Products shall be deliverd to contractors warehouse. 4. The specified products are to be installed at the jobsite no later than sixty (60) calendar days after receipt of executed purchase order. 5. All shipping material and debris to be removed from jobsite the day of installation and at no additional cost to the BVAMC. 6. No trash dumpsters at the jobsite belonging to the BVAMC or other contractors shall be used for this installation. Delivery and Installation: 1. Hours of Coverage: Normal hours of coverage will be Monday through Friday from 7:00am to 4:30pm, excluding federal holidays. Federal Holidays observed by the VAMC are: New Year s Day, Martin Luther King Day, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. 2. Delivery and installation shall be made to the Anniston/Oxford CBOC located at 96 Ali Way Creekside South, Oxford, AL 36203 Phone: 256-832-4141. 3. The job site does NOT have freight elevators or a pneumatic lift. The jobsite is a one story building in a busy strip mall. Offerors shall provide the information required by FAR 52.212-1 (APR 2014), Instructions to Offerors-Commercial Items, which is incorporated by reference. If the end product (s) is other than domestic end product(s) as defined in the clause entitled Buy American Act Supplies, the offeror shall so state and shall list the country of origin. FAR 52.212-2 (OCT 2014) Evaluation Commercial Items- The following factors shall be used to evaluate offers: Technical Capability of the item offered to meet the Government requirement Price FAR 52.212-4 (DEC 2014), Contract Terms and Conditions-Commercial Items is applicable.
