- COMBINED SYNOPSIS SOLICITATION This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. VA101V-17-Q-0372 The solicitation incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-95 This solicitation is set-aside exclusively for Service-Disabled Veteran-Owned Small Business concerns. To be considered for award prospective offerors must be a verified SDVOB concern in the VA Center for Verification and Evaluation (CVE) Veteran Information Pages (VIP) pages at the date and time set for receipt of offers. An Offeror who is not a verified SDVOB concern at the date and time set for receipt of offers will have their offer rejected as non-responsive, and will not be considered for award. The applicable NAIC Code for this solicitation is 238220 Plumbing, Heating, and Air-Conditioning Contractors Size Standard 14 million. HVAC Maintenance, see attached SOW. Veterans Benefits Administration (VBA) Indianapolis Regional Office (RO) Audio Visual System Relocation Statement of Work August 2, 2017 Background: The Regional Office requires relocation of the audio visual system from Room 331 to Room 347. Currently sixteen (16) televisions, located on the 2nd and 3rd floors have cabling run to seven (7) PC s located in Room 331. Cabling supporting the AV system is a mix of CAT 5/6e & Fiber Optic. CAT 5/6e is run to all displays located on the 3rd Floor with the exception of Room 362. Four (4) fiber optic drops are located in Room 331 and are run to Room s 362, 208, 222, and 226. All CAT5/6e runs from Room 331 are daisy-chained utilizing a TRIPP LITE CAT5/6e to VGA conversion boxes to display on the Divisions respective displays. Fiber optic drops utilize a Fiber to HDMI conversion box and ran to a one by four HDMI splitter. Seven (7) Dell OptiPlex 780 Core 2 Duo E8400 3 GHz currently run images to the displays located through the VA Regional Office. Each PC has a dedicated image pertaining to the Division in which the displays are located. Four (4) PC s currently transmit the image utilizing CAT 5/6e cabling; while three (3) transmit the signal from an HDMI cable to a HDMI to Fiber Optic conversion box. Currently, the three (3) fiber connected PC s are offline due to possible damage and/or malfunction. Room 347 contains three (3) rack housings, each containing components to different AV systems throughout the Regional Office. Existing racks have space to house a limited number of PC s split between the three (3) racks, however it is the desire of the Regional Office to have a new rack solely dedicated for the AV system PC s. Room 347 is approximately 80 feet from Room 331 in common space and approximately 20 40 feet straight line distance. Scope of Work: The scope of services includes relocating all components to the AV system. The Contractor shall provide all supervision, labor, administrative support, materials, tools, parts, supplies, equipment, and transportation necessary to effectively and efficiently fulfill all the requirements of the Statement of Work. The Contractor shall work closely with the Support Services Division (SSD) team. Requirements: The audio visual relocation agreement shall include: Conduct a detailed site survey Identify the cabling to be moved from Room 331 to Room 347 Removal of existing system from Room 331 Removal of existing system cabling from Room 331 Provide and install at a minimum three (3) Dell OptiPlex 780 Core 2 Duo E8400 3 GHz PC s with 8GB Memory, 250 GB Hard Drive Capacity, 1 HDMI Input, 3 GHz Processor; Intel GMA X4500 Graphics Processor, and Windows 10 Remove three (3) possibly damaged/ malfunctioning PC s from existing AV system Provide and install rack system capable of storing at a minimum ten (10) PC s Run all AV system cabling from Room 331 to Room 347 In the instance AV cabling is not long enough, run new cabling from Room 347 to the affected display(s) Install all necessary wire mold plugs Install all electrical outlets, network jacks and cable receptacles as needed to complete the installation Provide and install all additional peripherals necessary to ensure functionality of AV system Test all connections to ensure connectivity and functionality Provide cleanup of space to ensure immediate use Place of Performance: Department of Veterans Affairs ATTN: Horace Jordan 575 N. Pennsylvania St. Indianapolis, Indiana 46204 Period of Performance: 90 Days after Receipt of Award. Contract Administration of Task Order: The on-site SSD Supervisor(s) will serve as Government Project Officer (GPO). They will provide general instructions on limitations and deadlines, and is responsible for the administration of the task order in compliance with the contract to include inspection and acceptance of deliverables. Changes and modifications can only be authorized by a warranted Contracting Officer. Cancellation of Services: The Government reserves the right to cancel whole or in part services or the entire task order. Cancellation will be communicated and executed by issuance of a supplemental agreement modification to the Purchase Order in accordance with the terms and conditions of the providers. The following provisions and clauses are added as addenda: 52.252-2 Clauses Incorporated By Reference (FEB 1998) 52.204-9 Personal Identity Verification of Contractor Personnel (JAN 2011) Insurance Work on a Government Installation (JAN 1997) (a) Workers compensation and employer s liability. Contractors are required to comply with applicable Federal and State workers compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer s liability section of the insurance policy, except when contract operations are so commingled with a contractor s commercial operations that it would not be practical to require this coverage. Employer s liability coverage of at least $100,000 shall be required, except in States with exclusive or monopolistic funds that do not permit workers compensation to be written by private carriers. (See 28.305(c) for treatment of contracts subject to the Defense Base Act.) (b) General liability. (1) The contracting officer shall require bodily injury liability insurance coverage written on the comprehensive form of policy of at least $500,000 per occurrence. (2) Property damage liability insurance shall be required only in special circumstances as determined by the agency. (c) Automobile liability. The contracting officer shall require automobile liability insurance written on the comprehensive form of policy. The policy shall provide for bodily injury and property damage liability covering the operation of all automobiles used in connection with performing the contract. Policies covering automobiles operated in the United States shall provide coverage of at least $200,000 per person and $500,000 per occurrence for bodily injury and $20,000 per occurrence for property damage. The amount of liability coverage on other policies shall be commensurate with any legal requirements of the locality and sufficient to meet normal and customary claims. 52.232.40 Providing Accelerated Payments to Small Business Subcontractors (DEC 2013) VAAR 852.203-70 Commercial Advertising (JAN 2008) VAAR 852.219-10 VA Notice of Total Service-Disabled Veteran-Owned Small Business Set-Aside (DEVIATION) (JUL 2016) Limitations on Subcontracting - Monitoring and Compliance This solicitation includes VAAR Clause 852.219-10, VA Notice of Service-Disabled Veteran-Owned Small Business Set-Aside. Accordingly, any contract resulting from this solicitation will include this clause. The contractor is advised in performing contract administration functions, the CO may use the services of a support contractor(s) retained by VA to assist in assessing the contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to contractor's offices where the contractor's business records or other proprietary data are retained and to review such business records regarding the contractor's compliance with this requirement. All support contractors conducting this review on behalf of VA will be required to sign an Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement to ensure the contractor's business records or other proprietary data reviewed or obtained in the course of assisting the CO in assessing the contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs. Furthermore, if VA determines any services the support contractor(s) will perform in assessing compliance are advisory and assistance services as defined in FAR 2.101, Definitions, the support contractor(s) must also enter into an agreement with the contractor to protect proprietary information as required by FAR 9.505-4, obtaining access to proprietary information, paragraph (b). The contractor is required to cooperate fully and make available any records as may be required to enable the CO to assess the contractor's compliance with the limitations on subcontracting or percentage of work performance requirement.
