The 42d Contracting Squadron, Maxwell AFB, AL, is seeking sources from the NETCENTS II Products contracts that can replace the existing existing Maxwell clinic waiting room furniture. Request that all responses to this sources sought be submitted by 8 Aug 2017, 1:00 p.m. The 42d Contracting Squadron, Maxwell AFB, AL, is seeking sources from the NETCENTS II Products contracts that can replace the existing existing Maxwell clinic waiting room furniture. In accordance with the attached "draft" Statement of Requirements (SOR). NOTES OF IMPORTANCE: 1. A solicitation does not exist for this requirement. This is only a sources sought notice, so please do not submit proposals. The Government does not intend to award a contract on the basis of this notice. 2. If the government decides to pursue a solicitation, portions of your responses to this sources sought notice may be incorporated into the final solicitation. Respondents are discouraged from submitting any information deemed competition-sensitive or proprietary. 3. Respondents will not be reimbursed for any costs incurred in responding to this sources sought. Information submitted will not be returned to the respective respondents. 4. Oral submissions of information will not be considered in response to this sources sought. To assist the Government in developing a potential acquisition strategy, please provide the following information: 1. GENERAL INFORMATION: a. Company's full name, business size, address, point of contact and telephone number. 2. CAPABILITIES: Please address the following regarding your capabilities in meeting the requirements contained in the attached "draft" SOR: a. Is your company capable of providing a replace to the maxwell clinic waiting room furniture required to meet this requirement as specified in the attached Statement of Requirements (SOR)? Please respond "Yes or No." b. If your response to question #a or. above was "Yes", please provide the following information: c. Delivery Requirements: As a part of this sources sought, the Government would like to obtain the standard lead time for the delivery and installation of the products contained in the table above and the attached SOR. d. The existing Maxwell clinic waiting room furniture is reaching the end it's serviceability and is starting to present an infection control issue. The replacement of the furniture is required to continue safe patient operations. 2.0 DESCRIPTION OF ITEMS. The following items are required for performance under this contract. 2.1 Three Seater w/ Arms/ Legs(Quantity: 68) 2.1.1 Seats will have 3 back and seat cushions separated by arm rails. See example in Attachment 1 Figure A. 2.1.2 Seats will have 8 legs. 2.1.3 Seats must have antimicrobial ratings, comply with TJC infection control standards, and be cleanable with Bleach without altering the product. 2.1.4 Seat cushions must be moisture resistant and replaceable. 2.1.5 Seats must be spanned 2.1.6 Seats will be made of impact resistant polymer materials with durable edge guard. Various style options including, but not limited to, color, finish, texture, and edge/trim detail shall be provided for selection consideration. Final selection of style will be determined following contract award. Color and style options must be cosmetically appealing to the Medical Group. 2.2. Two Seater w/ Arms/ Legs (Quantity: 30) 2.2.1 Seats will have 2 back and seat cushions separated by arm rails. See example in Attachment 1 Figure B. 2.2.2 Seats will have 6 legs. 2.2.3 Seats must have antimicrobial ratings, comply with TJC infection control standards, and be cleanable with Bleach without altering the product. 2.2.4 Seat cushions must be moisture resistant and replaceable. 2.2.5 Seats must be spanned 2.2.6 Seats will be made of impact resistant polymer materials with durable edge guard. Various style options including, but not limited to, color, finish, texture, and edge/trim detail shall be provided for selection consideration. Final selection of style will be determined following contract award. Color and style options must be cosmetically appealing to the Medical Group. 2.3. Single Seater (Quantity: 64) 2.3.1 See example in Attachment 1 Figure C. 2.3.2 Seats must have antimicrobial ratings, comply with TJC infection control standards, and be cleanable with Bleach without altering the product. 2.3.3 Seat cushions must be moisture resistant and replaceable. 2.3.4 Seats must be spanned 2.3.5 Seats will be made of impact resistant polymer materials with durable edge guard. Various style options including, but not limited to, color, finish, texture, and edge/trim detail shall be provided for selection consideration. Final selection of style will be determined following contract award. Color and style options must be cosmetically appealing to the Medical Group. 2.4. Bariatric Single Seater (Quantity: 28) 2.4.1 See example in Attachment 1 Figure D. 2.4.2 Seats must have a width of 30" a depth of 19" and a weight capacity of 500lb 2.4.3 Seats must have antimicrobial ratings, comply with TJC infection control standards, and be cleanable with Bleach without altering the product. 2.4.4 Seat cushions must be moisture resistant and replaceable. 2.4.5 Seats must be spanned 2.4.6 Seats will be made of impact resistant polymer materials with durable edge guard. Various style options including, but not limited to, color, finish, texture, and edge/trim detail shall be provided for selection consideration. Final selection of style will be determined following contract award. Color and style options must be cosmetically appealing to the Medical Group.. 2.5. Round Table (Quantity: 130 ) 2.5.1 See example in Attachment 1 Figure E 2.5.2 Tables must be at least 20" tall. 2.5.3 Tables must have antimicrobial ratings, comply with TJC infection control standards, and be cleanable with Bleach without altering the product. 2.5.4 Tables will be made of impact resistant polymer materials with durable edge guard. Various style options including, but not limited to, color, finish, texture, and edge/trim detail shall be provided for selection consideration. Final selection of style will be determined following contract award. Color and style options must be cosmetically appealing to the Medical Group.. 3.0 INSTALLATION REQUIREMENTS: 3.1 The vendor shall assemble all furniture items. 3.2 The vendor shall be responsible for installation of new furniture and removal/haul away/disposal of old furniture. 3.3 All work and associated installation shall be complete and fully operational not later than January 5, 2018. 4.0 DELIVERY REQUIREMENTS: Furniture items shall be delivered and carried into the following address on a day and time approved by the Medical Group: 42nd MDG 300 Twining St Maxwell AFB, Al 36112 5.0 WARRANTY: Items will include at least a 10 year warranty. 6.0 GENERAL INFORMATION: Items must be brand new. Refurbished items are not authorized. Vendor shall be responsible for removal of all packaging and crating materials.
