THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR QUOTE. The NCO 12, Great Lakes Acquisition Center, 115 S. 84th Street, Suite 101, Milwaukee, WI   53214, is performing market research to determine if there is a sufficient number of qualified: (1) Service Disabled Veteran Owned Small Business (SDVOSB); (2) Veteran Owned Small Business (VOSB); or (3) Small or emerging small business firms, or (4) Large business firms, to provide four (4) Eyecon Model 9400, brand name only, counting devices, with software proprietary to SCRIPTPRO, for the pharmacies at the Clement J. Zablocki VAMC in Milwaukee, WI, the John H. Bradley Appleton CBOC in Appleton, WI, and the Milo C. Huempfner Green Bay CBOC in Green Bay, WI. General Requirements: BRAND NAME ONLY (EYECON MODEL 9400): Four (4) Eyecon Model 9400 counting devices, with software proprietary to SCRIPTPRO, shall include: Four (4) SP Dual Serial Interface Controllers. Two (2) Standard Interfaces. Installation (one at Clement J. Zablocki VAMC in Milwaukee, WI / one at the John H. Bradley Appleton CBOC in Appleton, WI, and two for the Milo C. Huempfner Green Bay CBOC in Green Bay, WI). Four (4) 12-Month, Monthly Support Services. SPECIFICATIONS: Operator color LCD display to provide information related to the product being counted. Camera system to capture the image of tablets and provide information to process the count. Counting tray on which to pour the tablets/capsules and visually count the medication. Barcode reader with provided database to provide validation of tablet size, shape and color. Dispensing port to remove medication from the counting device. Additional trays to use on which to count sensitive allergen producing medications. Ability to record and recover information for medications filled. Safety validation mode to ensure patient safety with confirmation of dispensed medication as well as separate counting trays for allergen producing medications. Speed Fast counting of poured medications of up to 200 milliseconds and updated pill counts to 5 times per second. Accuracy demonstrated accuracy up to 100%. Quality ability to recognize foreign matter or tablet fragments or un-uniform shaped product. Confidence Open design to allow for quick and easy inspection of the medication, backed up by a saved image of the pill tray for every transaction. Versatility counts all sizes and shapes of capsules and tablets. Ability to change counting mode from a validation mode to simplified counting, if desired. Interface with ScriptPro. Minimum platter surface area: 48 square inches. Maximum weight: 18 pounds. Minimum screen size: 6.5-inch color LCD touchscreen. Minimum USB ports: 2 per device. All interested firms who can meet the requirements stated above should respond, in writing, INCLUDING ALL THE FOLLOWING: Company name. Address. Point of contact (name/title/telephone number/e-mail address). FSS Contract number, if applicable. DUNS number. Tax ID number. Indication of which business category (SDVOSB, VOSB, Small Business or Large Business) for which the organization qualifies. A capability statement that addresses the organizations qualifications and ability to provide the requirement depicted above. Documentation from manufacturer (i.e. correspondence from manufacturer) of your firm being an authorized distributor for the manufacturer. Submit responses to the Contracting Officer, Thomas Chesak, at thomas.chesak@va.gov no later than 10:00 a.m. (Central Standard Time), 7 August 2017. VA69D-17-N-1504 Eyecon for Clement J. Zablocki VAMC shall be referenced on all correspondence regarding this announcement. TELEPHONIC INQUIRIES WILL NOT BE ACCEPTED. The NAICS Code 339112, Medical and Surgical Instruments, Equipment and Supplies, is applicable to this acquisition; the size standard is 1,000 employees. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Systems for Award Management (SAM) database prior to award of a contract. The Government will use responses to this notice to make an appropriate acquisition decision. This is the only notice; no other copies of this notice will be provided.
