REQUEST FOR QUOTE This is a combined synopsis/solicitation for Brand Name or Equal commercial item for twelve (12) oncology patient RECLINERS, prepared in accordance with the format in FAR 13.5 as supplemented with additional information included in this notice for the VHA Indianapolis. his announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. VA250-17-N-0560 is issued as a request for quotes (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-83. Contractor must be registered with https://www.sam.gov. The North American Industry Classification System (NAICS) number is 337127 or 337215 or other appropriate coding. Any contractor that believes they are capable and desires to claim preference for small business status must be registered with the SBA at http://web.sba.gov/pro-net/ and meet the requirements of FAR 19.102. Any contractor that believes they are capable and desires to claim preference for veteran owned small business status must be registered with the VIP at https://www.vip.vetbiz.gov as an SDVOSB or VOSB. This order may possibly be set-aside for SDVOSB and/or VOSB or Small Business, depending upon responses, the rule of two, and the NAICS waiver or exception, and if responses are applicable under the FAR 19.102 and 19.502-2, and VAAR 852.219-10 and 852.219-11. Description: ITEM NUMBER DESCRIPTION OF SUPPLIES/SERVICES QUANTITY UNIT UNIT PRICE AMOUNT 0001 002 Carolina Amenity Recliners or Equivalent Installation and Removal of twelve (12) recliners for the Oncology Clinics Twelve (12) EA Specifications: Carolina Amenity Sleep Recliner or equivalent, Brand or Equal item, for the Oncology Infusion Clinics. The Chemotherapy patient recliner will meet or exceed the following requirements: Standard width patient recliner with multiple reclining positions, to include full-flat bed position and backrest locks. Minimum chair measurements: outside dimensions 31.5"w X 39"d X 44"h, seat dimensions 24"w, 21"d, 21"high, seat to top of back height= 30", arm height 25.5", extended length 72". Steel recliner mechanism. Footrest and back operate independently of one another. Back can be infinitely positioned to any position angle. Upholstered arms (with optional poly arm cap), seat and back. Powder-coated steel foot release lever with Silver Ion Antimicrobial protection. 4" swivel front casters with brake and 4" fixed rear casters. Upholstery = Combination of solid/pattern from the following Momentum lines: Silica Solid, Silca Jig. Pattern and color combination TBD by VA interior designer. A completed copy of this provision shall be submitted with the offer unless ORCA registered which must be noted in quotation. 52.212-4 - Contract Terms and Conditions - Commercial Items. 52.212-5 - Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items to include: 4, 7, 8, 14, 22, 26, 27, 28, 30, 33, 40, 44, and 50. 52.252-2 - Clauses Incorporated by Reference.
