Description

(i) This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is VA250-17-Q-0964 and is issued as a Request for Quotation (RFQ). (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-95 (Effective January 13, 2017). (iv) This combined synopsis solicitation is Set-Aside for Service Disabled Veteran Owned Small Business (SDVOSB). The North American Industry Classification System (NAICS) code is 333314, and the size standard is 500 employees. Interested Service Disabled Veteran Owned Small Business offerors must be listed and registered in Vetbiz.gov to be considered for award. (v) The contractor shall supply the following Haag-Streit brand name only slit lamps and tonometers: Haag-Streit BM900 Slit Lamp with LED Slit Illumination, Base Model, Mfr Part No. 0900.0003LED (QTY: 5 Each) Haag-Streit BM900 Slit Lamp with LED Slit Illumination, Table Model, Mfr Part No. 0900.0001LED (QTY: 2 Each) Haag-Streit R900 Tonometer with 2 Prisms, Mfr Part No. 0900.2101 (QTY: 7 Each) (vi) To be technically acceptable the products quoted shall be brand name exact match. (vii) Contractor shall deliver all items 30 days ARO (After Receipt of Order), FOB Destination to the Saint Joseph County VA Clinic, 1540 Trinity Place, Mishawaka, IN 46545. (viii) 52.212-1 Instructions to Offerors - Commercial Items, applies to this acquisition. Addenda to FAR 52.212-1 Instructions to Offerors Commercial Items: Offerors shall e-mail complete quotes to Jeffrey.Rozema@va.gov no later than 4:00 PM EST on Tuesday, August 8, 2017. Responses shall be on an all or none basis; no partial submissions will be accepted. No grey market or refurbished items. The Government reserves the right to make no award in response to this RFQ. FAR 52.203-18, 52.204-22, and 52.211-6 apply. (ix) 52.212-2, Evaluation - Commercial Items, does not apply to this acquisition. Award will be based on lowest price technically acceptable offer. (x) 52.212-3 Offeror Representations and Certifications - Commercial Items, applies to this acquisition. The offeror shall complete only paragraphs (b) of this provision if the Offeror has completed the annual representations and certifications electronically via the System for Award Management (SAM) Web site located at http://www.sam.gov/portal. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (u) of this provision. (xi) 52.212-4 Contract Terms and Conditions - Commercial Items, applies to this acquisition. The following addenda apply: FAR 52.203-99, 52.219-28, and 52.232-40; VAAR 852.203-70, 852.219-10, 852.232-72, 852.246-70, and 852.246-71. (xii) 52.212-5 Contracts Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition. The following FAR clauses identified at paragraph b of FAR 52.212-5 are applicable to this acquisition: 52.204-10, 52.209-6, 52.219-28, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-36, 52.222-50, 52.223-18, 52.225-1, 52.225-3; 52.225-13, 52.232-33.
