Description
Statement of Work Blood Bank Reagent (Column Agglutination (gel) Testing) BACKGROUND: The New Mexico VA Healthcare System (NMVAHCS), Pathology and Laboratory Medicine Service (P&LMS) must have the ability to perform antibody screening and identification using the column agglutination (gel) testing in order to provide safe and compatible blood for our patients requiring blood transfusion. SCOPE: The New Mexico VA Healthcare System (NMVAHCS), Pathology and Laboratory Medicine Service (P&LMS), blood bank section requires a contract for a base year and four option years for FDA approved reagents and supplies to continue performing column agglutination (gel) testing in the blood bank in order to provide safe and compatible blood for patient blood transfusion. SPECIFIC TASKS: Delivery of the following estimated quantities must be made by means of a monthly standing order: Item # Description reagent/supplies Product Code Estimated Annual Volumes Unit 1 0.8% Selectogen 2x10mL 6902315 48 sets 2 0.8% Resolve A 11x3 mL 6902317 12 sets 3 MTS Anti-IGG Gel Card 5x20 MTS084024 13 boxes 4 Diluent MTS9230 2 boxes 5 MTS Tipmaster Pip Tip Bulk MTS9632 9 bags CORRELATION: Vendor shall provide the facility with sufficient reagents to perform correlation studies. Throughout the life of the agreement, the vendor shall provide assistance to the user for all troubleshooting issues and additional tests as needed. SERVICE: Vendor shall provide technical support. For reagent with hazardous constituents, vendor shall provide a mechanism for the laboratory to meet local discharge requirements. The vendor shall supply information regarding any hazardous chemicals (see Other, below). OTHER: In the event that any reagent is found to be defective and unsuitable for use, or the contractor has filed to comply with the requirements of routine supply deliver, the contractor shall deliver the reagent within a period of 24 hours after the receipt of the verbal order for priority delivery from the facility. If either circumstance has occurred, the vendor shall deliver to the facility, in the most expeditious manner possible, without additional cost to the facility, the necessary reagent in sufficient quantity so testing is not delayed. This contract should assist the NMVAHCS to pursue the goal of being a good steward in our community by eliminating, minimizing or mitigating adverse environmental impacts. The following chemicals have been targeted by the EPA specifically for reductions and feasible elimination. Please list any reagents (and in what concentration) that contain the following chemicals: Benzene Carbon Tetrachloride Chloroform Dischloromethane Methyl ethyl Ketone Toluene Methyl isobutyl Ketone Xylene Tetrachloro-ethane Cadmium 1,1,1-Trichloro-ethane Chromium Trichloro-ethylene Cyanid Lead Mercury Nickel PERFORMANCE MONITORING: The COR will ensure that product has been received for all charges listed on invoices. Blood bank will notify COR if there is any delay in receipt of monthly standing order. SECURITY REQUIREMENTS: This is for reagents and supplies only. The C&A requirements do not apply. The Security Accreditation Package is not required. GOVERNMENT FURNISHED EQUIPMENT (GFE)/GOVERNMENT FURNISHED INFORMATION: No government furnished equipment nor information will be provided to the contractor. RISK CONTROL: All cellular reagents will be handled with standard precautions. PLACE OF PERFORMANCE: New Mexico VA Healthcare System 1501 San Pedro SE (P&LMS 113) Albuquerque, New Mexico 87108 PERIOD OF PERFORMANCE: Base Year: 10/1/2017 thru 9/30/2018 Option Year 1: 10/1/2018 thru 9/30/2019 Option Year 2: 10/1/2019 thru 9/30/2020 Option Year 3: 10/1/2020 thru 9/30/2021 Option Year 4: 10/1/2021 thru 9/30/2022 DELIVERY SCHEDULE: Every 30 days
