Oklahoma City VA Health Care System (OKC VAHCS) Oklahoma City, OK 73104 Edstrom Watering System BRIEF SCOPE OF WORK (This is for information only) IMPORTANTR NOTE: This RFI (Request for Information) is for information only. No award will be made from this RFI. This RFI will be used as Market Research in order to determine the socioeconomic business size of any interested companies. Please communicate via e-mail to Lance.Davis2@va.gov by COB August 10, 2017 as to your company s ability to perform service per this brief Scope of Work. Please state your company s business size (please provide DUNS number) and if you have a FSS/GSA contract that includes this service. DESCRIPTION: VA Medical Center (OKL) is looking for maintenance and repair services for Edstrom Watering System. This system is used at the VA Medical Center, Oklahoma City, OK. Requirements: MAINTENANCE 1.1 The contractor shall provide services on Pressure Reducing Stations, Room Distribution System, Cage Wash Equipment and Pulse. Basic 1. Preventive Maintenance Services for designated equipment will be performed twice (2) per year. These include verification, calibrations, adjustments, repairs and/or replacement of worn/defective parts due to normal use and conditions. Parts charges are either included within the agreement or are invoiced separately depending on the selection from page 1. Purchase orders are required for any additional parts if not included in the Service Agreement. 2. The annual Service visit will be completed within 90 days of contract start date for all equipment detailed on the Scope of work page. Calibrations, adjustments, repairs, and applicable Preventive Maintenance services will be done on covered components. 3. The interim Service visit will be completed approximately (6) months after the annual visit for all equipment detailed on the Scope of work page. Calibrations, adjustments, repairs, and applicable Preventive Maintenance services will be done on covered components. 4. A Preventive Maintenance report will be completed for each visit and presented to an authorized company representative for approval. An original copy signed by both parties will be given to the representative. 5. 24/7 phone support Premium 1. All of the above 2. Additional service calls throughout the year between scheduled Preventive Maintenance visits with dispatching priority over Basic contract customers. B. Parts Replacement: 1. If selected, all replacement parts needed during the contract period to keep the system operating within specifications will be furnished except consumables such as printer paper and ribbons, filter cartridges, chlorine, etc., which must be supplied by customer. 2. Factory rebuilt boards (PCB) and components will be used at Edstrom's discretion. Defective/repaired parts must be returned to Edstrom for exchange unless directed otherwise by a designated Edstrom representative. 3. On contracts outside of the United States, the costs associated with shipping, duties, and/or taxes are not included in the price of the service agreement. In the United States, all covered shipping is Ground only. Extra charges will apply for priority/rush shipments. 4. This contract covers installation of parts during the Preventive Maintenance visit only. At all other times, the Customer is responsible to self-install necessary parts. If customer requires Edstrom to install parts outside of the normal Preventive Maintenance visit, the purchase of an additional service call is needed. PARTS 2.1 The contractor shall furnish all spare and replacement parts needed to service the equipment. The price of all parts used to perform the unscheduled maintenance shall be included in the coverage. Only new standard parts (manufactured by the maker of the equipment, or equal) shall be furnished to maintain the equipment. LABOR 3.1 All labor costs shall be included in the unscheduled maintenance proposal offered by contractor. DOCUMENTATION/REPORTS 4.1 The documentation of the unscheduled maintenance shall include detailed descriptions of the maintenance procedures performed, including replaced parts and prices required to return the equipment to its normal operating specifications. Each Service Report shall include, at minimum, the following: a. Name of contractor and contract number b. Name of technician or engineer who performed the services c. Contractor Service ticket number d. Date, time e. Description of problem reported, if applicable. f. Identification of equipment serviced: - Inventory ID number - Model and serial number - any manufacturer identification numbers g. Itemized description of service performed: - Labor - Parts (with part numbers) - Narrative of what was performed and checked. ACCEPTANCE OF CONDITION OF EQUIPMENT 5.1 The contractor accepts responsibility for the equipment listed below in an as is condition. COMPETENCY AND CERTIFICATION OF PERSONNEL SERVICING EQUIPMENT 6.1 Each respondent must have an established business, with an office and full time staff. 6.2 Fully Qualified is based on successful completion of factory training and experience in the field. For training, the technician or engineer shall have successfully completed a formalized training program for the equipment identified in the schedule. CONTRACTOR TEST EQUIPMENT 7.1 The contractor shall ensure all contractor supplied test equipment is properly calibrated, and certified. If requested, the contractor shall be able to supply any certificates tied to the quality of the test equipment. Test equipment calibration shall be traceable to the National Institute of Standards and Technology (NIST). Location of Performance: VA Medical Center 921 NE 13th St Oklahoma City OK 73104
