Lessor _______ Gov t. ________ Page 4 of 5 Page _____ of _____ August 4, 2017 SF-30 AMENDMENT NO. 3 SFO # VA101-16-R-0071 SAN ANTONIO, TX PART I BASIC SOLICITATION REQUIREMENTS SECTION 1.7 HOW TO OFFER - This section has been deleted in its entirety and replaced with the following: Proposals are required to be received by the Contracting Office by the date and time specified in Paragraph 1.5 of the SFO. In addition, a courtesy copy must be submitted to VA s broker by the date and time specified in Paragraph 1.5 of the SFO. The VA s receipt will serve as the official proposal submission for determining if the proposal was submitted on time. For proposals shipped, provide both VA and VA s Broker a receipt demonstrating delivery to VA is scheduled to occur before 4:00 PM ET on the specified date. Hand carried proposals must be coordinated with Kelli Witt at (202) 632-5706 to arrange for her to pick up the package in the lobby of 425 I Street, NW Washington, DC 20001. All hand delivery submissions must be completed before 4:00 PM ET. If pursuing Green Globes ®-NC, the Offeror may add GBI Green Globes ® Professionals (GGPs) to the project team, but it is not required. If one or more GGPs are added, the Offeror must identify any GGPs as team members, including their roles throughout the project. SECTION 1.10 DAVIS BACON WAGES - This section has been deleted in its entirety and replaced with the following: The wages to be paid during performance under this lease contract must conform to the Department of Labor s General Wage Decision No. TX170280, dated July 21, 2017, and as may be amended during the period of construction of the leased premises. A copy of the standards is provided in PART VII of this Solicitation. It is the Lessor s responsibility to obtain and maintain the most current rates. SECTION 2.14 SERVICE OF PROTEST (SEPT 2006) - This section has been deleted in its entirety and replaced with the following: (a) Protests, as defined in section 33.101 of the Federal Acquisition Regulation, that are filed directly with an agency, and copies of any protests that are filed with the Government Accountability Office (GAO), shall be served on the Contracting Officer (addressed as follows) by obtaining written and dated acknowledgment of receipt from: Tamim Chowdhury Contracting Officer Office of Real Property (ORP) Office of Construction & Facilities Management (CFM) U.S. Department of Veteran Affairs 425 I Street, NW, Room 6W210E Washington, DC 20001 (b) The copy of any protest shall be received in the office designated above within one day of filing a protest with the GAO. SECTION 4.2.1.B VA ADOPTED CODES, STANDARDS, AND EXECUTIVE ORDERS/MANDATORY PROVISIONS FOR ENERGY CONSERVATION/MEASUREMENT AND VERIFICATION The section entitled Measurements and Verification has been deleted in its entirety and replaced with the following text: Measurements and Verification: Per DOE Guidelines issued under section 103 of the Energy Policy Act of 2005 (EPAct), install building level advance utility meters in new major construction and renovation projects to track and continuously optimize performance. MOU mandates that the actual performance data from the first year of operation should be compared with the energy design target. After one year of occupancy, measure all new major installations using the Energy Star ® Benchmarking Tool for building and space types covered by ENERGY STAR ® or FEMP-designated equipment. SECTION 4.3.3 OCCUPANCY TYPE - This section has been deleted in its entirety and replaced with the following: Occupancy classifications are defined in NFPA 101 and as follows: NFPA 101: Use is an CBOC which is classified as an Ambulatory Care Occupancy Sedation anesthetics will be utilized in the facility. The facility is to be designed to meet all local, state and federal codes that regulate an Ambulatory Care Facility. SECTION 4.8.1 GREEN GLOBES CERTIFICATON This section has been deleted in its entirety and replaced with the following: All references to Green Globes are hereby deleted from the SFO, schedules and addenda. This facility is to comply with Energy Star and all local energy codes. SECTION 7.2 PARTITIONS The second paragraph of this section has been deleted and replaced with the following text: Use 5/8-inch thick gypsum wallboard ASTM C1396, except for special conditions. Use fire resistant Type X or Type C wallboard ASTM C1396 in fire resistant rated assemblies. Use moisture resistant wallboard ASTM C620 at wet locations. Provide accessories, fasteners, and finishing materials in accordance with ASTM C1047, C1002, and C840. Install and finish gypsum wallboard in accordance with ASTM C840. Use Level 5 finish texture for all public corridors and lobby areas with paint finish. Provide a Level 4 finish behind all other areas to receive a paint finish. Provide a Level 3 finish for surfaces to receive any type of vinyl wall coverings. Provide a Level 2 finish for surface to receive ceramic tile and in rooms or spaces for which no decorative finish is specified in Schedule E. SECTION 10.4 COMPLETION OF GSA FORM 3518 - This section has been deleted in its entirety and replaced with the following: COMPLETION OF GSA FORM 3518-SAM The Offeror must submit with the offer a signed copy of GSA Form 3518-SAM, Representations and Certifications, with all information completed as requested. FAR 52.204-8 Annual Representations and Certifications. (Jan 2017) (a)(1) The North American Industry Classification System (NAICS) code for this acquisition is in the below table. Please ensure your response includes the revised, updated forms below: Pt 07--Labor Standards - San Antonio TX Rev. 07-28-17 PART VIII FORMS The following form has been deleted from this section: Pt 08G(1)--GSA3518 - May 2013.pdf (Note: Offerors are still required to provide Pt 08G(2) GSA Form 3518-SAM, Addendum to the System for Award Management (SAM) Representations and Certifications PART X SCHEDULE F The previous file has been deleted and replaced with the following revised file: Pt 10B--Schedule F - San Antonio TX Rev. 07-28-17.xls Note - Schedule F Revisions: There are (3) Washer/disinfector sterilizers (S0960) in Schedule F specified for the Reverse Osmosis room. These sterilizers are now to tag to the Soiled Manual Equipment Wash CWSH1 Room instead. These sterilizers are currently specified to be VA furnished and installed. They are to be changed to be specified to be VA furnished and Contractor Installed. There are (4) Terminal sterilizers (S0405) indicated in the alcove just off of the Clean Preparation room that are NOT currently specified. Add these (4) to Schedule F and locate the sterilizers in room Clean Preparation Area CSIA2. Changes to Schedule F are highlighted in yellow. The LOGCAT legend has been added to this Addendum to supplement the responsibilities legend provided at the end of Schedule F.
