- Attachment 3 - Evaluation Criteria Attachment 2 - PWS Attachment 1 - Combined Synopsis/Solicitation Info SF1449 Solicitation Attachment 1 - Combined Synopsis/Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 and additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a separate solicitation will not be issued. This procurement is issued as unrestricted under the NAICS 611430 with a small business size standard of $11M. The Defense Logistics Agency (DLA) Contracting Services Office (DSCO) intends to purchase critical thinking assessments on behalf of the Defense Contract Audit Agency (DCAA). Please refer to the performance work statement (PWS) at Attachment 2 for complete requirements. The resulting contract will be a single award with a one year base, and two one-year options. The period of performance for the base year is September 15, 2017 through September 14, 2018. Price will be evaluated inclusive of option pricing. Award will be made on the basis of lowest price, technically acceptable (LPTA). In order to be determined technically acceptable, quotes must meet the minimum technical requirements identified in the attached evaluation criteria (Attachment 3). A government representative(s) will evaluate the information provided to determine if, in his/her opinion, the minimum requirements and intended use of the product(s) is met. The government is not required to research information outside of what is provided in order to make the determination. In addition to a price quote and completed solicitation form 1449, the following shall be provided: completed FAR and DFARS provisions 52.212-3 and 52.209-11, taxpayer identification number (TIN), Dun and Bradstreet number (DUNS), and Commercial and Government Entity (CAGE) code. Companies doing business with the federal government are required to use the System for Award Management (SAM). In order to be eligible to receive an award for this requirement, vendors must have an active SAM registration. The website to register, update, or re-activate an account at SAM is https://www.sam.gov. All questions and requests for information regarding this Request for Quote (RFQ) must be received NO LATER THAN close of business August 17, 2017 in order to minimize the need to extend the closing date. All quotes must be submitted through email to kelly.clements@dla.mil by the date/time listed in the RFQ. FAR Provision 52.212-1 is hereby modified as follows: Delete paragraph (d), Product Samples.
