Description
2 The Gulf Coast Veterans Health Care System (GCVHCS), Network Contracting Office (NCO) 16, Biloxi, MS, is issuing a combined synopsis/ solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice in conjunction with procedures FAR Part 13. We intend to issue a sole source purchase order to Tampa Armature Works (TAW), Inc., 6312 78th Street, Riverview, FL 33578-8835. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation is being issued. The solicitation number VA256-17-Q-1195 is issued as a Request for Quotation (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95 effective January 13, 2017. The NAICS code is 335312 and the size standard is 1,250 employees. This requirement is for a sole source and will be firm-fixed-price. The closing date for this notice is 12:00 PM CST Aug. 9, 2017. NOTE: This RFQ has a short notice for response time, three days only. The requirement is for the following: The GCVHCS is looking to award a one-time firm-fixed contract for the period of performance beginning from date of award until completion of services. The contractor shall furnish all labor, tools, materials, transportation, equipment, parts, testing, inspections and supervision. The equipment to be covered is a John Deere Generator, 275W Model 275RJC6DT3. (See Scope of Work below for detailed information). Contractor to provide services in accordance with terms and conditions of the contract for the date of award through August 30, 2017, or sooner. To be eligible for award of a contract, vendor must be actively registered in the SAM database (https://www.sam.gov). Vendor is also required to complete representations and certifications found in the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items. EMERGENCY REPAIRS TO REBUILD BUILDING 25 FIRE PUMP VA GULF COAST VETERANS HEALTH CARE SYSTEM BILOXI, MS 39531 SCOPE OF WORK Emergency repairs to replace high pressure fuel pump on Bldg. 14 emergency standby power generator VA Gulf Coast Veterans Health Care System, Biloxi, Mississippi. Work for this repair/rebuild project shall include all labor, tools, materials, transportation, equipment, parts, testing, inspections and supervision. Generator is a John Deere 275KW Model 275RJC6DT3 Installation Time All work shall be completed within 30 calendar days after Notice to Proceed is received by the Contractor. New work will be performed during the hours of 7:00 AM and 4:30 PM, Monday through Friday, exclusive of Federal Holidays. Some work outside of normal hours of operations may be necessary and must be coordinated with the Project COR. Special Considerations by the Contractor: All disposal costs and fees for debris removal will be the contractor s responsibility. All work performed shall comply with NFPA 99 requirements and if necessary the contractor is to supply certifications and system testing as required. Safety Considerations: Contractor s workmen shall provide and utilize all safety requirements in accordance with OSHA regulations during all phases of work. NOTE: Site shall be kept clean and free from debris at all times and contractor shall remove debris daily from medical center property. Contractor will not be allowed use of government materials and/or labor for this project. All work is to be performed in strict compliance with these specifications.
