Description

The Department of Veterans Affairs is pleased to announce the release of the attached request for quote (RFQ) for the repair of smoke/fire barrier deficiencies for various areas throughout the Wilkes-Barre Veterans Affairs Medical Center (VAMC), located at 1111 East End Blvd, Wilkes-Barre PA., 18711. This RFQ is set-aside for small business concerns. Please email your quote to Mr. John W. Evans at john.evans15@va.gov no later than noon Eastern, 16 August, 2017. Please contact Mr. Andrew Cullipher at (570) 824-3521 Ext. 7468, or email him at andrew.cullipher@va.gov for site visit information. It is advised that all interested vendors attend the site visit to see what fulfilling this requirement entails (Don t just send a prospective subcontractor). It is also advised to thoroughly review all attachments which contain pictures, schematics, and other relevant information. Please prepare your quote consistent with FAR provision 52.212-1 Instructions to Offerors/Commercial Items in paragraph E.1 of the RFQ. Pay particular attention to the following from that provision; (b) Offers shall be submitted on the SF 1449; (4) Provide a technical approach that shows a basis of design to correct deficiencies on door latching and how it will be integrated into alarm system. Also provide basis of design to correct barrier deficiencies (specify your intended method of correction). (6) Complete Price/cost schedule in section B; (10) Provide performance information to include three (3) recent and relevant contracts for the same or similar items and other references (including contract numbers, dollar amounts, periods of performance, points of contact with telephone numbers, POC email addresses, and other relevant information). Please email questions to Mr. Evans no later than 2:00 PM Eastern on 11 August, 2017. Questions received after that date and time may not be answered.
