- DTAR 1052.232-7003 clause IR1052.232 IPP Clause Clause IR1052.209-9002 This is the solicitation for the requirement. Please complete the Section B - line items and grand total. This is the statement of work for cable services. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. The Solicitation Number is TIRNO-17-Q-00219 and is issued as a Request for Quote (RFQ) with the statement of work. The solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular current to 2005-87. Period of performance includes a base and four option years. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) FAC 2005-88, effective 15 June 2016. This requirement is being procured using FAR Part 13, Simplified Acquisition Procedures. The associated North American Industrial Classification System (NAICS) code for this procurement is 515210 with a small business size standard of $38.5M. The government intends to issue a Firm-Fixed Price (FFP) award based on the lowest priced technically acceptable (LPTA) quote. The following factors shall be used to evaluate offers: technical capability and price. Offerors shall provide the required installation of equipment, maintenance and television programming services, (1) Installation of receiver, (2) Antenna system, (3) Amplifier, (4) Power supply, (5) RG11 coaxial cable and labor, (6) Emergency and regular services calls on per call basis, (7) Labor and materials for new cable outlets, and (8) Yearly television programming subscription of local news channels 4 (WRC), 5 (WTTG), 7 (WJLA), 9 (WUSA), 26 (WETA), 32 (WHUR), CNN, CNBC, MSNBC, FOX News, C-SPAN, C-SPAN2 and C-SPAN3 Quote submittal must, at a minimum show: (1) solicitation number, (2) price, (3) cable provider, (4) name, address, and telephone number of offeror, (4) Taxpayer Identification Number, (5) CAGE Code and (6) DUNS Number. Depending upon the submissions received, IRS will set aside a period of time to conduct a site visit. (This would take place in the downtown Washington, DC area and be entirely optional at the submitter's discretion and expense.) Vendors will receive email communication regarding the site visit. This solicitation requires registration with the System for Award Management (SAM) in order to be considered for award, pursuant to applicable regulations and guidelines. Registration information can be found at www.sam.gov. Registration must be "ACTIVE" at the time of award. All questions should be submitted to faith.m.ashton@irs.gov by 12 p.m. on August 9, 2017. Responses will be provided as an amendment to the solicitation on August 11, 2017. The RFQ closing date is 12:00 PM Eastern Standard Time, August 22, 2017. Vendors must complete and send the three provisions, IR1052.232-7003--Electronic Invoicing Payment Requirements for the Invoice Platform (IPP) (Aug 2012), paragraph ( c ) Contractor Point of Contact Information, IR1052.209-9002-Notice of Consent to Disclosure and Use of Taxpayer Return Information, IR1052.209-9002 Notice and Consent to Disclose and use of Taxpayer Return Information and DTAR 1052.209-71--Representation by Corporations regarding a unpaid Federal tax liability or conviction of a felony criminal violation under Federal Law (Deviation 2015-00002) (Jan 2015), paragraph (b)(1)(2) that is included in the RFQ as well as a separate document. Any quote received after the due date and time will not be considered and therefore be ineligible for award. No phone calls will be accepted.
